There are some aspects that are staged. But most of it is "real" in the sense that those people are actually there struggling. But an example of staging, in one episode a team of 4 had to make a raft and paddle two miles out into the ocean for rescue. A whole bunch of dried and cut bamboo just "happened" to wash up on the beach right next to where they're camping
