What Types of Prostate Cancer Patients Should Choose Active Surveillance?

  • Active surveillance is generally for patients that have lower grade prostate cancer because those cancers tend to grow more slowly. But, there has been a debate for a number of years about the over- and under- treatment of prostate cancer, and whether some prostate cancers need to be treated at all.
    The short answer is that not all prostate cancers will benefit from treatment, but physicians have to make sure that they are not under treating prostate cancer because it is very difficult, or impossible, to go back in time and take several years back when that patient actually had localized prostate cancer.
    The goal of prostate cancer treatment is getting those patients early enough so physicians have a much better chance of curing those patients.

    Good information! When I was diagnosed the MRI showed one mass about 8mm Gleason 6, biopsy showed cancer. They urged active surveillance, I was going to go with that but about 6 months later decided against it, and the next doctor wanted a new MRI and it showed a second mass at about 10mm, while the first mass was up to about 12mm. And the doctors said “that shouldn’t happen, you don’t have small cell carcinoma”. Needless to say I had it treated and am cancer free.

    My Gleason score is 4+3 and 3+3 and have not gone over 7.5 PSA reading monitored every two months.

    hi doc. we need you medical assessment regarding the condition of the prostate cancer of my father. after having confirmed thru a biosy result that our father has it, we were not given explanations as to what's the stage of the cancer, is it agressive or what. instead we were just given a prescription of meds he must take. please engliten us. thank you for your time and kindness.

    so if I don't listen too you 90% living 15 yrs, why the hell would I go thru the pain and taking the chance of you speading the cancer with a biopsi

      This interesting point you make, prostate cancer recurrence has been recorded up to 18 years.
      When the 3+3 and 3+4 (Epstein 1 & 2 grade) are the vast majority of the “cured group”, then one might say “take out the stage 1 & stage 2 cured, then treatment has no cures”.
      That’s also known as the Will Rogers effect.

      Biopsy don't spread..they track though
