Answer: D. Down syndrome. The fetus has the classic "double bubble" sign of duodenal atresia. The increase AFI points to polyhydramnios, which occurs due the the inability of the fetus to swallow the AF. Duodenal atresia is strongly associated with Down syndrome. GDM and Rh isoimmunization can both lead to hydramnios, but are not related to duodenal atresia.

Maternal DM causes increased blood sugar in the mother which in turn results in increased blood sugar in the fetus which causes osmotic dieresis, therefore causing hydramnnios.
