If we take connecting flight & lay over is in different country, then do we need to take visa for that country also( just check out & check in flight change over)..?
There are 2 conditions in this thing 1. If your airline is same & they are providing transfers, then no need of taking any kind of visa. 2. If you are booking different different flights. in this case you will claim your baggage and also for that you have to clear immigration and for that you will need visa for that country too.
For Malaysia & Thailand, is health insurance required for senior citizen? You said, bank statement required for Visa. If we carry sufficient amount (Ringgit/Bhat), then we need not carry any proof right?
They can ask you for bank statement It is recommended to carry for last 6 months. And also health insurance is not mandatory. But i suggest you to purchase 1. Kaafi sasta bhi hai nd kaafi kaam bhi aa sakta hai

There is no charges for visa as of now for both countries.
2 pin adapter works in both country. No need to buy another adaptor

There is no such cross check with the bank statement as well as card
