John, When I attempt to apply the first axis I do not get two points to pick on the upper and lower part of the hole. I have only one grip point.

I have seen this also on some models. When I do, I select the one point and then select the inside face of the hole. You also can select any face or plane that is perpendicular to the axis you desire.
