Tbh you gotta make it malformed in an uncanny way, such as larger than normal mouth, etc etc. As an artist it's kinda hard to describe it, but it takes a lot, uncanny, near unproportional changes to the physiology, along with many things can create real nightmare fuel. But shit like pibby, hell no, all it is is some stupid glitchcore art.
