Karta Pobytu(trc)/FILLING the APPLICATION FORM

The residence card is a residence permit issued to foreigners who have been granted in Poland:
temporary residence permits;
permanent residence permits;
long-term resident's EU residence permit;
residence permit for humanitarian reasons
international protection (granted refugee status in Poland or subsidiary protection).
The residence card, during its validity period, confirms the identity of the foreigner during his stay in the territory of the Republic of Poland and entitles, together with the travel document, to multiple border crossings without the need to obtain a visa.
Additionally, the residence card is a residence permit which, apart from a valid Polish national visa, entitles its holder to travel within the territories of other Schengen Member States for a period not exceeding 90 days during each 180-day period, if the foreigner:
has a valid travel document,
can justify the purpose and conditions of the planned stay and
has sufficient means of subsistence or the ability to obtain them legally,
and
is not considered to be a threat to public policy, internal security, public health or the international relations of any of the Member States, and in particular has not been issued on that basis for an alert for the purpose of refusing entry in the national databases of the Member States.

Hi! The last pages are dedicated for the personal of Urzad, so you don't need to fill them. So you need to fill only the ones I showed on the video :)
