Hence we usually explore other things like treatment compliance , or if you have a new illness (like TB) or if you have a problem with absorbing your pills( e.g diarrhoea) . Pregnancy shouldn’t have an effect on your CD4 as far as we know, and if there’s any changes in your viral load we look for other possible causes. Because we want to keep your viral load suppressed in order to prevent transmission to the baby, people who are not suppressed on TEE we change to TLD because it suppressed VL quicker than the old first line. If compliance is any issue or there’s any other reasons why your VL is climbing then we try and explore those reasons extensively and manage them asap. But if you are not suppressed on any other regimen (especially second line) it’s every important to look for other common reasons. But pregnancy shouldn’t be the reason why your viral load is climbing, in fact that’s when you want it more suppressed. So it’s not really common from my experience. 2. U=U (undetectable=untransmitable) has been proven by many studies, one of them being HPTN 052 trial. They have shown that it can be used as a “prevention method” to try prevent HIV transmission especially in serodiscodant couples. The CDC also supports the term because in all those studies, none of the virologically suppressed partners transmitted HIV to their partners. However the partner who is positive should remain suppressed. So according to these studies, one would say there’s evidence that prep is not really necessary If your partner is suppressed and remains suppressed. Prep is most preferred for people who are highly exposed to hiv transmission. Some partners still prefer to take prep even when their partners are suppressed , and if they fit the eligibility criteria with no contraindications we still let them take prep because of patient preference. But if your partner remains suppressed, studies have shown that U=U is a valid preventative method.
