RAI Therapy is OLD,SAFE,effective now used as a first-line treatment rather than surgery. permanent and reliable cure for hyperthyroidism. I-131is given which EMITS RADIATION. It destroy thyroid cell.

What equipment is used?
no equipment used during radioactive iodine therapy.
HOW DO I PREPARE FOR MY TREATMENT ?
no special preparation.remain fasting on the day of treatment.
Thyroid should be working when this treatment is taken, so antithyroid drugs, should be stopped before the treatment
Carbimazole, stop three days
Propylthiouracil, stop at least 2 weeks
Continue with your other medication
beta-blockers
in pregnancy RAI is not given.
stop breast feeding
take a 3- 5 days leave from work
low iodine diet for a few weeks
How is the procedure performed?
The treatment and its effects will be discussed
consent to the treatment.
I-131 is swallowed in a capsule or liquid dose
It is concentrated from the blood by the thyroid gland, where it begins destroying the gland's cells.
a single dose is successful in treating hyperthyroidism.
rarely, a second treatment is necessary. A third treatment is very rarely necessary.
HOW MUCH DOSE WILL BE GIVEN TO ME
Dose is very important as
to determine your dose - radioactive iodine uptake scan is done
larger the gland, the larger the dose.
higher the iodine uptake, the smaller the dose.
POST RAI CARE
return home following treatment.
DO I NEED TO FOLLOW SPECIAL DIET AFTER TREATMENT ?
Yes. low iodine diet
avoid prolonged, close contact with other like children under five years and pregnant women for 5-7 days
keep at arm’s length for two to three weeks
drink plenty of water.
excess will be excreted in saliva, sweat, tears, vaginal secretions, and faeces.
Use private toilet facilities.sit down when urinating.
flush the toilet 2-3 times after use
Wash hands and Bathe daily
Use disposable eating utensils.Launder clothes separately during first 3 days
avoid public transport
avoid prolonged automobile trip with others.
Do not prepare food with bare hands.
do pregnancy test within 24 hrs prior to RAI. avoid becoming pregnant from six months to one year after treatment.
breastfeeding women should not receive RAI therapy
avoid fathering a child for at least 4 TO 6months
carry a medical letter
Side effects of radioactive iodine?
Neck tenderness and swelling, Nausea.
Swollen salivary glands, Loss of taste ,Dry mouth ,Dry eyes.
Excessive tearing from the eyes.
Permanent hypothyroidism or underactive thyroid
Thyroid pills-inexpensive.reliable,safe ,doesnt impair fertility.
HOW LONG DOES RAI TREATMENT TAKE TO SHOW ITS EFFECT ? up to six months
How many doses needed - 90% need one dose
10% need 2nd dose.
no problems with having more than1 dose.
How will I know if I need 2nd dose
If symptoms persist after six months, you may have to receive a second dose.
Contraindications
Pregnant woman, Lactating women, women planning for pregnancy in 6 to 9 month time.
overactive thyroid can cause thyroid crisis or storm.
thyroid eye disease
to minimise the risk of worsening eye disease, steroid tablets, early treatment with levothyroxine and stopping smoking.
hypothyroidism can occur within 12 months

@@RKS442 because RAI worsens thyroid eye disease also i have goiter . So I think surgery is better for me
I have seen your video. It will be helpful for patients. One thing I want to highlight you, the term you used Technician is wrong. There is RSO (Radiation safety officer)involved with physician. There is no qualification for technician in Nuclear Medicine department. All are working as a Nuclear Medicine physicist or RSO. Technicians are working in radiology. For both department qualification is different. Kindly correct this.

hi. my wife have hypertyriod for the last 9 year. and now we are not sure if we should take the either radioactive iodline or surgury. but my point is either way. the person will became to hypo after treatment and will take medication for life. then what is the point to take these two treatment? we can stay on current hyper medication cabemizole for life too？
Thats the confusion most people are gng through but let me make clear with it... see its better to have hypo than hyper cause it can come up to the normal level after the medication taken for certain period whereas hyper can only be controlled for a particular time until n unless u take medications
If u gap medicine for a day or two u will notice the problems in hyper cause the hormone becomes active
n the medication which u take on hyper has much side effects which can result bad in future
Hyper symptoms ( high heart beat, eyes bulging, weight loss etc )are more dangerous than hypo so better to be in hypo.

Go to endocrinologist. I had same and did Radioactive iodine because my hyperthyroidism came back. First it was normal blood work and I did it again and it was hyperthyroidism. They give me heart medication and I did Radioactive iodine and I'm glad I did.

Hello doctor, I'm hyperthyroid patient. Age is 28 years and married. No children yet. I'm planning to have child after 6-8 months. I have diagnosed hyperthyroid 3 and half years ago. So I have been taking neo-mercazole from 3 and half years. From last one year my dosage is reduced to 5mg. In March 2022, my thyroid became normal, so my endocrinologist reduced the tablet to 2.5mg for two months, then again thyroid got abnormal, all T3, T4, Tsh became completely abnormal with 2.5mg. So my doctor again increased the dose to 5mg for 3-4 months, then thyroid readings became normal. Again doctor reduced dosage to 2.5mg for two months, then thyroid readings became abnormal. This experiment was done throughout 2022 year. I'm now thinking I have wasted one year by this experiment😢. Finally doctor suggested me to go for RAI in Jan-2023, but I have taken second opinion with another endocrinologist. He said that no need of RAI for you as you're taking not more than 5mg from the last one year. So he asked me to give him 3 months of duration, he said we can stop medication with neo-mercazole tablet only in the duration of 3 months. So he said me to wait for 3 more months before taking decision to undergo RAI. From last two months thyroid is normal, He is examining me for every 4 weeks. Right now I'm taking the Same tablet but 10mg dose on alternative days. Am I on the correct medication? Should I continue with this tablet only? or should I go for RAI?
My age is already 28 years completed. My confusion and fear is If I opt for RAI, RAI is good and effective alternative when it is done only in one dose. Suppose it is not the case, if doctor advise me to take second dose of RAI then I may need to postpone pregnancy for 2 more years. I can't wait for 2 more years with these complications( bcz I have Fibroadenoma too in breast).
What should I do now?
And one more question.
Which is better option among
1. Getting Pregnancy by changing medication to propylthiouracil tablets?
2. Going for RAI and become free from hyperthyroidism before planning to get pregnant?
Which is the better option for me. Please suggest me.

No. You will then be hypo.

Hi, Mam, I have Graves disease with hyperthyroidism since few years.My FT3 and FT4 is normal but tsh is 0.01.Doctor advised me for RAI therapy.I have bulging eye issues and problem of closing my eyes.after the RAI therapy, will that worsen my eye condition. Could you please advice.

Mam I'm 22. In October 2021 I tested FT4 , TSH and various test and doctor told me I have Hyperthyroidism and it was at 7 . I am taking Carbizol and Indever from October continuously. But every month I go for follow up with these blood tests. And it's sometimes 0 , sometimes 2 and sometimes 4 . Doctor told me he will give me RAI that two drops . And other doctor told me maybe it can turn to Hypo later in my future . I went for the other doctor with my tests and prescriptions for cross checking that's it. And I want to know that Will RAI cause any fertility issues in my future ? I'm not married .I'm an unmarried girl. I'm 5 feet and maybe 56 kg wieght . Mam I'm very worried about this treatment . Can you assure is it safe and it will not be harmful even having kids in future ?
No it won't. But u need to wait for minimum 6 months before u plan pregnancy

Good evening mam... My age is 34 ...I went through thyroid test... So my report values are T3 is 1.26 and T4 is 7.03 and TSH is 0.0100..my doctor told both T3 and T4 are good within reference values but TSH is not good.... So u should not plan for pregnancy. But as per my age I'm thinking to plan pregnancy... Bcoz TSH value is never coming normal reference value... I hv hyperthyroidism n using carbimazole 15 mg per day... Can u plz suggest mam abt my pregnancy plan....
