    Is this a patient who did not improve with PT, thus the orthopedic referral or was he evaluated and referred straight to ortho? If the latter, why not attempt conservative treatment first? And thank you for the content!

    I have the exact same problems as this patient after a traumatic accident. My orthopedic doctor is getting me some EMG testing as he suspects nerve damage.

    I am a patient. Based on my observation, he has Rotate cuff tear. PT will not work. Need surgery to fix it and then PT. I wonder what has happened now after a year ? Any update?

      I had three patients last year who had complete rotator cuff terrors who were able to avert surgery with physical therapy. Not every single person who has a rotator cuff tear needs to have surgery, but many do. There are a considerable number of factors that help determine if you should have surgery or not, this guy did have surgery

      @OrthoEvalPal Thank you! I am the one who has a partial tear. Supraspinatus. Had 3 months PT, slight improvement, but far away from normal. Pain comes and goes. I finally decided to have surgery. Now 12 weeks post op. Going back to PT again. I have the confidence to get my normal life back in the near future.

      Varicella zoster, the chickenpox virus. It’s a retrovirus, meaning it lives in the DNA of your cells forever, and during times of high stress or immune compromise, you can have an outbreak. It never crosses the line of summery on your body, and it’s a very painful attack on the nerves that is a accompanied by rash.

    It's looks like a rotator cuff tear but are you sure that the orthopedic will refered him back to you if surgery is not needed

      I have actually seen 3 people with mri diagnosed RCT's in the last two months who were scheduled for surgery and were able to cancel it. Doesn't happen often but some will do well despite the cuff tear.

      ​@OrthoEvalPal hello doc, couple of days ago i played armwrestling and now i have some loss of strenght on pull ups and a dull ache in the front of the shoulder.I have no loss of range of motion,but less strenght on internal rotation.I thought its okay to workout,but now its a bit worse. Do you think some ice, 2 weeks rest and a couple of tb 500 injections will fix it?
      By the way pull ups aggravate it way more than push ups.Any idea?
