Summary:

This is exactly the threat a 'mad' unicorn could pose. There was no Smarty Pants other than a lifeless doll. This was ALL Twilight, or rather something dark in her mind. Superb voice acting and editing. Also those rapid image changes were really good. The 'THREE' and Twilight on the verge of going feral.

My headcanon for this story. Twilight finally snaps and is overcome with schizophrenia, due to the lack of sleep (Yea I know, It's probably unlikely in a real world sceniaro). Twilight then unwillingly kills off Spike and Owlicious due to her current state of mind, and then because of her schizophrenia, links the voices in her head to her Smarty Pants doll. Thus explaining why the doll is audible to just Twilight, but not anypony else.

ACTUALLY! That’s not so farfetched! While schizophrenia can’t occur due to sleep deprivation things that CAN are:
Hallucinations, mania/psychosis, depression, withdrawal, etc.
So while it isn’t schizophrenia the sleep deprivation could DEFINITELY factor into her behaviour!
That... was... the most... SCAREIST... grimdark fic I've ever listened to. IMShadow does an excellent job as Twilight as always. Anna was great as Rarity. Reverb's music... just awesome and Dusk Spark's artwork was great and scary as hell. Scribbler was just great as ever, but this is the SECOND TIME she scared the living shit out of me. The first time being her Halloween special last year.

You know Lost, Shadow's great voice acting made me want to hold Twilight safe in my arms when she cried about Smarty pants on her head refusing to let her leave. Poor Twilight was so scared and who can blame her.

At the time I was interested in fallout equestria and aside from crazed rambling's narrations I couldn't find anyone with an even halfway decent skills. I had given up. Then I found your channel. Guys I am not kidding, along with the background music and Twilight's Glare @24:23 I was completely shocked and I could not look away from Twilight's glance. It was a very scary experience for me. Like, I just could not move. This is definitely one of the scariest things ever. The voice acting and everything is very well done. You certainly know how to edit the sound and capture the atmosphere.

The first time I heard this I assumed that smarty pants had been possessed and twilight had been tortured by the death of spike and snapped, just accepting smarty pants as her one true friend. Only on the second listen did I realize why some people were talking about psychosis and smarty pants being purely in twi’s head. I like that it can be interpreted multiple ways

When I hear this, I keep imagining Paranoid Twilight speaking to Smarty Pants. But all that other stuff. Ugh, gave me creeps.

My theory is similar to another commenter’s but with some alterations. I think Twilight has always had some form of mental illness looming in her mind (in this case most likely schizophrenia or some psychotic disorder) and some event, plus the lack of sleeping due to studying; has triggered it to flare up. I think this event is hinted by her being at sweet apple acres/having a conversation with Applejack at the start of the story. She actually stole Smartypants from Big Mac’s trunk herself while he was out for the day, maybe in an attempt to relive her younger days or to quell the stress/flare up of emotional problems. Her bringing this doll home, which is clearly hinted to have traumatized her in some way in the past, caused her to go into psychosis. In a manic state, she killed both Owlowiscious and Spike. This caused even more of a snap and that’s when she began to hear Smartypants talking to her - her own brain trying to convince her that she didn’t do the horrible things that she did. With the only other remaining “member” of the household being Smartypants, she latched on to it and that explains the “friendship” between them that Rarity witnesses when she visits Twilight.
Even if twilight is insane, and she's completely snapped, she should know a talking doll shouldn't exist. Even with dark magic, that really shouldn't be a thing, and she should know this.
Can we appreciate the badass heavy metal music at the end of the story i loved that
