The person, who broke Penny's neck, was shorter. His hands and arms were bigger than the Freak. It was obvious that Penny knew the person approaching her. She didn't feel threatened by this person. Penny would have likely ran away from the Freak

Not sure if this a subtle hint that he's not all there or not, but we had people in the nursing home I worked at who seemed perfectly fine, very well spoken, sociable etc, however, in reality they were HIGHLY confused but they could fake it REAL well especially with new staff or visitors.
