Carbon’d up inside and apparently roll out didn’t stop it? Burned inside and out. Hope you will have an easy time removing the boiler from the basement.
I think one of the inner flue passages got clogged due to rust chunks at the hole thru to the water side. Products of combustion build up at the burners, causing the fuel/air mixture to go rich and also flame roll-out. Just a theory.

I don't think there is a roll-out safety switch on this boiler.
