If you're playing Rock Band:
Red: MIDI note 38
Yellow: MIDI note 46
Blue: MIDI note 48
Green: MIDI note 45
Kick: MIDI note 36
If you're playing Guitar Hero:
Use the same MIDI notes as you would for Rock Band, but add Orange: MIDI note 49.

+Marcus Young This edition of GH has a means to reward stars beyond 5, so it's very easy to rack up 5 from just standard playing. I think the most I earned was over 20. Instead of stars awarded based on points, it's stars based on accomplishments (streaks, SP duration, etc)
+Morxis but that only happens on the power challenge. You can get only six stars in normal play

Power Challenge allows up to 21. 21 if you FC the song and already have 20 Nova Stars...
Typically, high note streaks, tons of points and high multipliers will get you there (but thus far on my instrument of choice I've only gotten 21 Nova Stars in one Power Challenge and that was on 2112 Pt 6 - Soliloquy by Rush - I played Expert Bass using Lars and Pandora's Level 2 Powers which are Multiplier Extender and Star Power Nova respectively).
The closest otherwise is my 20 Nova Stars with Dethklok's Bloodlines on Expert Vocals using Echo's SP Generator and Pandora's Star Power Nova.
Of course, outside the Power Challenge, you can gain additional stars for instrument and full band challenges such as High Score, SP Score, Up/Down/Alt Strumming, Specific Note, Duration of Multiplier at x4 or other variable, among others.
A lot of Bass Challenges are Up Strums, some have Alt Strums.
Probably the most insidious of the challenges is the Bank SP.
It's Amon Amarth. It's a part of the Death Metal DLC.
