I think there are a couple of scenarios. I can see Karim grabbing Rachel when Sakina is in labor or after the baby is born by saying he will carry the baby tell the mother to come to the car I am waiting. Or after baby is born Karim can brutally attack Sakina and while she is recovering as the father he takes off with Rachel and new baby. In fact if Sakina is in the hospital he would probably use the house. She might be in a coma, not expected to survive. Should be interesting.
