That one where you asked if it was Petrol... it looked like grain and if that truck was running, any kind of spark with the grain and dust will cause a fire. Just grain dust alone is very flammable.

I worked at a grain elevator that blew up because someone standing in the door way lit a cigarette while we were transferring grain and the air was full of dust. It used to be that working in a mill especially a flour mill was one of the most dangerous job the average Joe could have. You start grinding and the dust flies and it’s just like having the air full of gas fumes. And when you are using a crane you have to center the load or it will over balance the crane. The more off center the load the less you can lift.

The second vid with the spilling grain is a dust explosion.
You don't hear about it much anymore, but grain silo explosions used to happen on a more than occasional basis. Dust generated by kernels of grain (corn, wheat, barley, etc) grinding against each other causing a dried grain dust. Since the dust is from plants, dried, it's highly flammable. If ignited (generally a spark) it explodes if contained, as if in a silo. But, if it's out in the open, it "foomfs", much as gasoline vapors ignite when using gasoline to start a fire.
I'll bet the driver in the loading shed was scared shitless, but I'll also bet he was unharmed.
If he had his arm resting on the truck windowsill, he might've singed the hair on his arm. But it happens so fast that it's usually over before the victim/participant can even react.
But it looks spectacular.@4:54 the faling shelves, I don't think that's Idiots at Work. When the store buys crappy display shelves because they're cheaper, this is what you get.

You're so funny. As soon as you said in your intro that industrial accidents don't happen too often, I started laughing. My dad was dispatcher for a large interstate trucking company, and as such anything that went wrong for any of the drivers was Dad's mess to clean up. I can't begin to tell you how many family dinners we didn't have because of the way those guys could screw up a delivery, or driving into a weigh station, or loading their trucks improperly and therefore being unbalanced. Sometimes Dad would get the call in the kitchen while we were eating dinner (where the phone was) and we'd hear him say something like, "I'm sorry, Officer, but he did WHAT?" Then he'd roll his eyes and nod to my mother and she'd grab his version of a "go bag," and off he'd go.
As for the crane operators...my niece just married a guy who operates one of the super cranes. My sister had mentioned he made great money and that he'd been head-hunted to move clear across the country. He's known for being really good at his job and after watching a few videos like these you can see why he's worth his weight in gold.
One last comment. My best friend was using the PA Turnpike to navigate her tri-city area. As she was zooming along (people are serious about exceeding the speed limit on this stretch, especially, so you either keep up or stay off that part of the highway. And lots of people drive long distances to avoid it!) a large dump truck with its bucket fully extended, went flying by her. They were approaching an underpass, over which a four lane highway went. The whole time she was saying, "it's not gonna fit! Stop!" Of course, he went right on, slamming into that bridge causing hundreds of thousands of dollars in damages. Cars on the bridge were jolted, causing fender benders up there, and there were a couple behind the truck, too, because of the speeds they travel on that stretch of road. The only thing that saved that guy from jail, most likely, is that no one died. Of course, it took hours to open up that extremely busy part of the Turnpike, a Bad Thing in itself.

That was very likely grain that exploded. This happens all over the world. Grain, or sawdust, or other combustibles can become aerosolized (sp?) and explode. Remember, ignition requires only three things: Oxygen, fuel (ground corn, for example) and a spark. An amazingly sad occurrence happens at outdoor concerts when dancing crowds are sprayed with colored cornstarch. While this floats through the air it is beautiful. Then the cornstarch reaches a cigarette or joint. The result is flaming young people burning to death.

The answer to your last question is NO!!! Let's say the avg. European country (due to size) has 35,000,000 people in it and 65% is of working age, and 10% of the workforce is incompetent. That's 2,275,0000 people that have the possibility to screw things up. The USA has roughly 330,000,000 people and with the same working age percentage, that comes to 21,450,000 possibilities to go wrong. Thing is, this number still has all the liberties of the remaining 90% to walk around society as a whole.

I can't help but wonder if that truck driver in the segment at 2:20 is going to be held responsible for the damage to the helicopter. Granted, it's cheaper than replacing the _whole helicopter,_ but depending on the model, that can cost over $200,000 _per blade._ So that's potentially over half a million dollars of damage...
The segment at 5:10 is obviously a failed demolition. Ideally, the building should have collapsed on top of itself (controlled implosion), but believe it or not, it could be worse; it only took a few temporary buildings out. Imagine how bad the damage would be if it'd fallen the other way, into one of the other towers behind it. At least now they don't have to finish the job the old-fashioned way (with wrecking balls).

Often times, I'm not saying it's always the case but, bigger machinery built for "1st world" nations find their way into "2nd and 3rd world" countries, but are in questionably working condition. The idea's pretty straightforward, but there's no manual and no training, so the support legs on a crane, those make nice seats or are annoying bits to be cut off. And why not use a crane to lift a crane to get more work done? It's because the formal training isn't there. Also time is money, so any time long safety procedures can be cut or shortened, they are. Mr Ballen has plenty of stories about when that all happened.
But often times, it's just that the job manager is an imbecile themselves who has been trusted by corporate imbeciles who thing giving complex jobs to underqualified people will save them money. By saving them money by paying people less than they're worth, they then get middle management getting less than qualified "grunts" doing dangerous tasks with the bull s**t promise of, "It's a really easy job, you just have to do this/that/all of these things." They don't seem to realize that when you tell someone put ship in bottle without any context, they'll find a way to put the ship (at least the materials of the ship) into a bottle. You didn't specify or train them otherwise so when you get your bottle filled with wood and string encased in glue, that's what you asked for.
But then again, there are genuine screw ups people have and then the screw ups people do to try to cover up their initial screw ups with absolutely 0 knowledge or reason beyond they're trying to not get into so much trouble, like the guy who knocked over all the electronics shelves. However things like the guy knocking over the entire brick wall is something more a long the lines of both miscommunication (no on telling him the wall wasn't set) and probable lack of training (the guy looked like a manager/middle management who's never done that kind of job.)

It's not unfortunate, it's just physics! It's why we always over spec on everything, especially if it going over someone's head. Pretty much all the rigging I've ever used has been at least 5 to 1, if not 10 to 1 (except non-consequential, low-load things). Here though, I suspect it was the attachment point or shackle that let go first, adding ALL the additional weight to the other sling. I think that's the metal ping right at the beginning of the fall, and the loud metallic thud at the end. Side-loading a shackle would make those 2 distinct sounds. This I know from 'I was there, not my rigging' experience.
