In this scenario you were the seller; however, is this a form what you would create as well for sending out all offers that were accepted via a purchase agreement? After the purchase agreement is signed, then you could send via notary a land contract?
You'd have to clarify what you mean by land wholesaling. If you mean buying land very cheap and selling it with seller financing, yes... but you'll want to be sure you have an attorney prepare the first one in whichever state you're working in, to make sure it's got all the state-specific verbiage in it.
I would google your state and city and the words "real estate attorney" and call a few of them to get their opinions. Most attorneys will give you 15 minutes of their time without charging anything.

I suppose you could - you'd just want to make sure all the language is written in such a way that you're still okay with it.

What do you do if the parcel has no address?
Just refer to its parcel number and legal description (most vacant land properties don't have a registered address, so this is a pretty common thing).
