Here's what I think. You see how viruses spread by infecting .com or .exe files, right? Plus, most viruses don't show operate unless it's a certain date. When an infected computer uses floppy disks with exes or coms on them, the programs on the floppy then get infected. Then, the virus spreads via infected floppies to new computers, and from infected computers to new floppies.
Mainly piracy was super easy. Like just stuff a game in a floppy and chuck it. Chances of you getting a virus FROM the guy who gave you the game on floppy are high. And the guy who gave you that virus probably got infected as well. (Viruses on dos used to infect all the files). So it then becomes a huge cycle where like everyone on the suburb street/block has the virus. Before one can warn the Uninfected. It's probably infected their PC and the game with the virus has been sent to like 5 more friends, it's an unstoppable virus machine thing until one smart kid breaks the floppy and stops the virus flow and alerts his friends.
Usually when someone who had access to a modem downloaded a pirated game/program infected with a virus, (yes, downloading files from Internet was possible even during the DOS-era, through BBS, FTP or Usenet for instance,) copied it to a floppy, gave it to a friend, who gave it to a friend and so on. And that's how you'd acquire a DOS-virus, because you wouldn't even know you were infected until the virus activated since the anti-virus program Microsoft supplied was pretty much useless.
