I have 2 axolotls that do the mouth thing on a regular basis, they do close their mouths most of the time however, I've taken both to get checked out and both are completely ok, they usually do it before a yawn as well, so I'm assuming if they do it ALL the time then yes it is something wrong perhaps a stuck piece of food or infection, though infection would also have other symptoms so watch or other unusual visuals as well

as an adult they can come out of water for a small amount of time but you shouldn’t be taking them out of the water anyway and should never be taken out of the water.

You should never ever remove an axolotl from its tank unless absolutely necessary (taking it to the vet, for example). And even then, they still need to be in water so they can breathe properly.
Axolotls are not supposed to be pets. They are very active but once you put them in a tank they become stress and they hesitate when you feed them. In Mexico they are sacred to us.
Be sure to research how to take care of them if you do get one! They're not complicated animals, but they're still a responsibility you need to take care of.
