Who is a candidate for migraine surgery? The most important thing to identify before taking some of a migraine surgery is to identify their trigger points. Every migraine patient is not a candidate for migraine surgery. Firstly they need to have a trigger point which is identified. Secondly they need to have tried other treatments first such as medications. Migraine surgery is not something you do right after that. So first point how do you identify someone who has a trigger point well we take a detailed history. We ask the patient where their migraine start How does it go what is its intensity how frequently does it happen we ask them to keep a migraine log which is a record of all their migraines for at least 2 to 3 weeks and then we examine that log and that helps us determine the trigger point is significant. The other thing we can do is that if someone is having an active attack of migraine then during that active attack we give a little bit of local anesthetic injection into their trigger point. And if this local anesthesia injection reduces the migraine when it is given that is a very good indication that this person can be a good candidate for migraine surgery. The other thing is to make sure that they have tried everything else that they are taking medications and they have seen a neurologist again. Like I said this is not a the first thing you do when someone has a migraine those patients who have been suffering for a long time who get migraines almost like 3 to 4 days a week who are severely incapacitated by this and they have identifiable trigger sites are the ones who will respond best to this surgery.
