Lochte went one step further by being on his back. That's ingenious.
Just to clarify: the turn WAS NOT illegal. It became illegal only some years ago, but when he did it, it was totally legal, that's why he was not disqualified. The rule was introduced only after all of these swims.

The rule is not well thought out. Doing a dolphin kick chest down is the same as the butterfly underwater. So, by the logic of the current rule, that should be illegal as well.

Backstroke is any stroke swum on the back, including while underwater. It's illegal to do any kick underwater after the turn to the freestyle portion if the kick is while the swimmer is past vertical towards the back.

The rule you’re thinking of only applies in IM. You can kick on your back in freestyle

It's more complicated than that when it comes to the freestyle part of the Medley... read the rules carefully (reminded in the comments)
