Planning for dental implants at the front of the mouth is notoriously difficult.
We need to take several things into account, from the level of infection around the tooth to what technique should we use to minimise bone volume loss when the tooth is removed.
Excellent, informative presentation. Wish you were near me. I have to undergo major (I think) incisor implants soon. I just wish I could have total anaesthesia and wake up with it all done.

I’m looking for advice on dealing with black triangles around an implant crown, a problem I’m facing after the crown was placed a few years ago. I’ve found plenty of videos explaining that these triangles occur due to a lack of bone, and I have seen different options about fixing the issue. Some videos suggest options like gum grafting, using fillers, or altering the shape of the crown.
They have already adjusted the shape of my implant crown, but it now looks bulky and noticeable.
What would you recommend for achieving a more natural appearance when black triangles are present on both sides of an implant crown?

I'm having an implant and bone graft soon under local anaesthetic and I'm so worried about the potential pain during the procedure.
I just had a front tooth implant put in & also bone grafted , procedure took 1 hour, some pins were hit in to hold the membrane I think that was the worst part , a lot of stinging afterwards but later that day felt much better with co- codamol 30mg pain relief & ice
My body clears anaesthetic really quickly, but the bone graft and implant were fine. It was no more unpleasant than a large filling. Hope it all goes/went well.
How did your procedure go? I've just come back from the dentist (I haven't been since the Covid outbreak) and i've received some very sad news, that i have gum disease and one of my front teeth will very likely have to come out, I can't feel anything there and the nerve is dead. I kinda walked back home in a daze not looking at the traffic and almost got hit by a car but I was wondering caring, I just crawled into bed and had a little cry. Then I remembered I need to be at work, so i showered up and I'm about to leave to go to work now. I don't know how i'm going to live with myself with a front tooth missing, it's going to completely destroy what little confidence I had to begin with. Can i ask what tooth are you having an implant from please?
You need to be careful doing too much in one go. It is easier for one small thing to go wrong and then it does not work out in the end. These problems do not occur overnight and it is wrong to expect the solution to be overnight. Sometimes as another option to what you mentioned it may be better to erupt the bad tooth bringing the gum down and then extract and place the implant same visit. I would not load it at time of placement and find another way for a temporary.
So after tooth extraction, the bone can grow back? And then the bone grafting isn’t needed?

I have gum pain around crown that was put on a couple of months ago. Will getting it replaced cause further damage to the root and gums for the front tooth?
Interesting thank you. I broke 2 of my front teeth when I was a kid and now have a denture for one. Unfortunately they fitted them incorrectly and it has push my gum up. I’m hoping to get an implant when I have the money I suspect I will need some kind of graph to build it up.
So you have the same dentures? Did they not change them as you grew older? Or, were the last lot not fitted correctly?
@@emmasanders6226 i had crowns fitted as a child but it wasnt till i had got older that that the tooth had to be removed. When i went for my follow up appointment after fitting the denture they didnt say anything was wrong. It eas picked up by a private dentist when i went to asses the cost of an implant. They actually fixed the denture for me but said i would probaly need a graph for the damage caused. I returned back to the nhs dentist to complain but they said it was too late and also the other dentist had gone.
@@nrosko Wow that's terrible. NHS should be held accountable regardless if the dentist is no longer on the payroll. In saying that the disclaimers they have in the fine print probably protects them.
in about 5-10 years I will need to get this done, how much do I need to save lol
It cost about $4000 dollars
why pay 3k for implant when i can get all my teeth sorted out abroad? UK dentistry is either rubbish or expensive or both!!!!!!

@@drgurssehmi no luck is needed, just good research. stop insinuating that dentistry abroad is dodgier than in the UK.
One implant in the USA is $5000. And no guarantee it will work. It could still fail. All depends on how good your dentist is.
I had a front tooth implant in Costa Rica, removal of broken tooth at the gumline immediate implant $1300.03 months later I went back for the crown 450 Emacs full ceramic crown in America I would’ve been out $9000 for one tooth

Dentist here are useless. They don't even rinse your mouth when they're drilling to do a crown/filling and put that horrible chemical to clean where the nerve was. One dentist sprained her wrist doing an extraction. Both.of my front teeth were cracked at the roots and I was told I would need them pulled out or they would rot and infect the teeth beside them. . That was 8 years ago. I still have my front teeth and the ones beside them except one front tooth is now starting to get a cavity on the tip. Too many horror stories that I have little faith in dentists. You sound nice. Our dentists could learn a lot from you in attitude alone.
you have a BSc degree in Dentistry. Why the term "Dr."?

@@drgurssehmi in your country, the "Dr." is reserved for those with a doctoral degree (requiring a dissertation).
Yes, I think you should write a formal letter of complaint. To someone. I'm not sure who is going to care though.
@@jn3750 We have Dr's in social science I discovered at one point and all that time I thought Dr. was a shortened version of doctor. 🤣🤣 There are many of those Dr's that do not have a doctorate degree either

I've recently started using a socket shield/bone graft with good results. The idea of immediate placement and load is tempting but when one fails it is very disappointing. Great work on these videos. I'm an implant dentist in the states.
Agreed - and they always fail when the patient forgot about them and ate something hard!

Hi Dr.Sehmi, I was thinking of to get your advice. I have a slight separation of tooth#8 and Tooth#9. I had an Apico on tooth#8 in April of 2019, and the tooth has resorption and I was thinking to just have the tooth extracted and an implant put in to match up close to tooth#9. I had no separation of the 2 teeth only till around a year ago. I also need an implant of tooth#31 since I had the tooth removed because of an infected old root canal. Just wondering your advice? If space is issue, for front tooth implant, can Orthodontics create space, I already had ortho done earlier and teeth removed due to that. Pls answer, thanks

Bit worried because I was told it may appear big however it’s the opposite😢 the tooth is smaller😢 and not flush with my big front tooth.🥹does the “dental implant temp Crown” shift or change, during the next weeks (4.5 wks to be exact) that I have to wait until I return to the for the permanent crown?
