No matter how many videos I watch or tutorials I do, I cannot get this down to be a short, non painful aspect of the job. I'm so frustrated with myself as it is making me a very inefficient clinician and I'm tired of my lack of progress in this area. I'm so frustrated.

It is a really frustrating task for sooooo many therapists! And it may always be annoying, but it CAN get easier. Like laundry and dishes, notes will always pile up each day if we're not on top of them. However, there are strategies to make them more simple. Also, self-compassion is key!!! Without self-compassion, the struggle will never cease.

All well and good until you stand in coroner court explain what your notes mean and them ripping to shreds how ambiguous they are

True, the point is not actually to make your notes ambiguous. It's to make them personalized to the individual client, but to do so more easily.
