Back in the OlD dAyS sugar was an extreme luxury that only the rich ever had small amounts of. The sugar feeds the bacteria that lives on our teeth and makes them produce acid that damages our teeth, feudal japan would have no need of toothbrushes and toothpaste because the overwhelming amount of peoples diets consist of rice, fish, assorted vegetables and the like. The only access to sugar people would have is fruit.
