- One of the biggest fears with men who are about to undergo surgery for prostate cancer is the loss of sexual function. This video gives a brief overview of what to expect, based on my own experiences.

My prostate removed in june 2017 then 33 radiation treatments at prostate bed 5 months after removal- still incontinent 3 thick pads a day, if I starve myself of water then less, no erections, it sucks. still undetectable psa as of 3 months age. 59 yo at removal.

My husband had one of the best surgeons in the country and after a year post surgery can not get an erection and has urinary incontinence. You were lucky.
Age is a huge factor. You look like a young fella. Lots of factors to consider when it comes to expectations. Anyone watching should realize there are a lot of differences between people, which could be related to age and what damage was done during surgery. This is one guy telling his story, not everyone's.
I had a prostatectomy 3 years ago at age 58. No erections in three years. Penis dead as a doorknob.
I was thinking the same. I'm really happy for this guy but it would seem for most you're at the mercy of the surgeon's skill.

Thank you for the videos, really helped me to prepare my body for surgery. I am 12 days post surgery .I had my KAD removed and my continence is very good. I am still sored, but I am hopeful that my sexual function will be back. I feel a little growth down below but not full erection yet. Part of my nerves was removed due to the position of the cancer. Mark......Thank you for all of your help!!! I am 57, and have no cancer, and has had laser ablation surgery....and had the uro lift procedure.....and the prostate just keeps on growing back!!!! Because of looking at the procedure online, and viewing your experience, I think that we will talk to my surgeon to see if I am a candidate for this procedure!!! It's been three years ago since your comment. Can you tell me what you ultimately did and how it worked out? I had TURP two years ago and now I'm right back where I was before. I don't know what to do.

Thanks for your video, it did help me a lot. I started pelvic floor and body core exercises 5 weeks before surgery and the result was I'm dry 100% from day one this was my biggest worry.

My friend's husband did prostectomy in Jan or Feb 2022 she said he is half his previous size and no cialis or viagra works. Its sad😥but he gas always been a good man to her he would never leave her if the case was reversed but its really sad

I think the advice in this video is amazing. First of all his guys gotta share about his personal sexual life with random strangers? What does he get out of this? Anybody questioning this man needs to take a look at their own lives and asked, “what am I doing to help other people“?
I’ve never heard the tip about taking small doses of Cialis after surgery and I completely understand the logic behind wanting to stimulate yourself and let your brain know that just because your prostate was removed that you have not given up on sex. That’s something extremely valuable that I had not heard from my surgeon or any other support staff

Most men don't talk about it because most men don't have sexual function after prostrate removal such was my case it's been a year also still partially incontinent I don't have cancer but I don't have my old life back either I guess I can't win them all this is one man's experience not everyone take it with a grain of salt.
I agree. I suppose that some men have normal sexual function, but I believe that a large percentage do not, and a lot of men lie about it to avoid embarrassment. I had the robotic prostatectomy in 2008 and have not had sexual function since. To this day, I still have some minor incontinence. The good news is that my PSA is still zero, so I guess it was a good trade-off after seeing my father die from prostate cancer in 1990.

Mark, I am happy that you have had a successful outcome. Unfortunately many men do not have the same experience that you have had. Remember, urologists are surgeons and as such, they do prefer to cut. There is an alternative to taking out the prostate and that is a series of radiation treatments. I have gone through 42 radiation treatments and a brachy seeding procedure with no incontinence issues. Sexual function is not back as I am still on hormone treatments for the next 6 to 8 months. After that, I have been told sexual function should return. If not there are several other solutions.
The radiation is terrible I spoke to a lot of men that had this procedure now they wish they never had.

Get the penis injection! Three different meds in one shot in the penis! And get ready to party!
Dec 20 made a year. Only thing that works in the injection. I took 3 vigra and nothing. Lilttle pieces daily and one right before try to have sex. Then I start pissing on her talk about kill the mood.
Had my prostate removed in 2016 at age 62. Waved bye bye to cancer…..intercourse then too.

Great, simple, clear video. I never heard an explanation of the "saving the nerves" technique, about the net of nerves around the prostate (that control erection) and how many surgeons simply cut the net open, remove the gland, then "reconnect" the net, uh-huh, and the patient winds up impotent. My urologist was reluctant to recommend any treatment, probably lawyer shy, but if you get the standard radiation treatment, impotency will happen. That may be described as the ability to penetrate. In retrospect, I would fly to New York and hook up with the surgeon this man identifies.
Thanks for the down to earth information. I was diagnosed 3 days ago and this is very scary stuff. I have to go for a cat scan and bone scan now. This is all very overwelming and a tough road ahead. I guess right now I`m mostly worried the cancer has spread to other places. My doctor has been monitoring my PSA levels for almost 2 years. So by his experience he had me do a biopsy 3 weeks ago. It came out positive 5 out of 12 cores the agressive type of cancer. He is moving fast with my treatment and has many appointments sceduelded for the next while. The video maker is one of the lucky ones. Everything he says about the sensation is true. My Doc just prescribed the Cialis. Can't wait to see how it works. Maybe the trick is to start it right after surgery instead of later on.
My PSA was 14, gleason was at 5. Biopsied tissue post op was an 8. The damn thing started to spread very fast. It was even in the seminal vesicles.
He told me had I not removed it when I did there was a good chance it would've spread to where it would be incurable.
God is good man, all the time. We need a support group cos I'm 60, a grandfather a year ago she have no desire to make any more kids... But it fkkks with me knowing that I CAN'T even if I did want to. There are down days and some outright depression some days. I have to keep reminding myself I'M A CANCER SURVIVOR.

Hi buddy, I have not had it done yet. I had to reschedule the operation due to issues. However it will be a removal of the Prostate. This is now scheduled for the 2nd of August. I'm hoping for two things. It has not spread and the nerves are spared. I am having it done in Thailand at a private hospital there. The robotic surgery will be done.

Thanks for sharing Mark! I just hit year five after my surgery. I was 45 at the time. My surgeon was amazing, and specializes in the nerve sparing technique. I got my first full erection 2 weeks after surgery without even trying which surprised my surgeon a bit. I do still use sildenafil ... I don't really need it. But with it my erections are like I'm 20 again so I will continue to use it. When flaccid, my penis does actually seem a little smaller than it was before the surgery, but my erections are full size. Ejaculations are mostly dry, however I do still produce a complete bucket load of precum which makes me happy because at least something's coming out of there. I've had instances where I've expelled urine during orgasms but that has gotten better as time has passed also... Kegel exercises help with that tremendously! I do still have some pain on occasion during my erection though that has gotten much better over the years. And one positive side effect of my surgery was that the hyper sensitivity you had mentioned in your video partially continued all this time later. There are parts of my penis that are much more sensitive now than they were before my surgery so I'll take that as a win! The one thing I will say to close is I know this is scary for any man that has to have this surgery, but it doesn't have to be bad. You're alive and healthy which is the most important thing, but you can still have a fantastic sex life, it just might take a minute to get there. But guys, don't forget your kegels. They are not just for women! They strengthen your pelvic muscles and give you way more control down there.

I have exactly 30 days till my surgery, and i'm not looking forward to the 'lack of the ability to get a hard-on' aspect. December 19th. My prostate is bulging on the one side and once removed the penile nerve goes with it. 50% chance of getting it up anymore. Oh well, at least I'm still alive......for now.

I enjoyed your video and am going to review the others you have posted as well. I totally understand my mileage may vary.
I am two days post-op (robotic assisted laparoscopic prostatectomy). I consulted with a Johns Hopkins trained surgeon and a well-respected radiation oncologist. I kept my wife in the loop throughout. My diagnosis was 5/12 cores positive with one core having a gleason score of 4+3 with perineural invasion.
I decided on RALP because of my age and the fact that it becomes exceedingly difficult to remove the prostate after cancer returns after radiation treatment. I've read about the potential ED and incontinence issues. I feel like I considered all possibilities and made an informed decision.
My surgeon initially said he was going to take the right nerve bundle. But, once he started the procedure and saw what he was dealing with, he spared both bundles. So, at this point, I'm hopeful that sexual functions will return to a sufficient degree. I will get the full path report in a week at my follow up appointment.

I am 5 weeks post surgical and went in with no expectations. The biopsy had shown cancer everywhere and it was an aggressive form. The gleason score was 8. My PSA level was being monitored (my persistence) and doubled in 4 weeks and doubled again 3 weeks. I go again in 3 weeks and hopefully it is falling or zero. That said I have leakage if I fart..so I try to make sure I am in a bathroom to fart. That said 2 nights I woke up soaked, wet the bed. I suspect I farted in my sleep and pissed like a rain bird sprinkler.
Each of our surgeries was different somewhat and not everyone has nerves in the exact configuration of another. Most will not have your results. Many would be happy to have mine. After being told the cancer was everywhere and aggressive I am relieved to be alive.

So, I have a few issues with this video. Speaking as a 52 year old male who had a robotic prostatectomy done by a very skilled surgeon on July 31, 2019 with a VERY HIGHLY functional sex drive before the surgery, Mark's promises of everything will be "just fine" ain't doing it for me. My surgeon was very skilled, had aprox 1000 surgeries under his belt and was told me that he was able to carefully and successfully spare both nerves before removing my prostate and said that everything went very well and that things should return to normal. I started manual manipulation and the penile pump therapy 8 days after surgery, almost daily so I started VERY early on and took this process VERY seriously. I take 20 mg of Sildenafil daily and twice a week take 100mg doses along with my pump process and still basically have nothing. Perhaps I've not given this enough time as some studies suggest that this can take 18 months to 36 months to fully recover however I notice that things down there do not look normal color-wise when pumping so I am continuing on this journey and hope that things will eventually return to normal.
Bottom line: Marks "supposed" success (and I DO hope that this is genuine as I wish no ill will on anyone) seems a bit far fetched and just be careful that you don't fall too deeply for his claims and "promises" for you. It's one of THE most important decisions of your life and you may not experience the same results. Thankfully I am still cancer free so that's the most important thing I suppose. Thanks Bradley for warning others about recovery after RP. In 90% of cases, it is not what Mark comments on. I was also young 51 and in excellent shape and was 100% potent going intoRP surgery.
According to my doc, it was successful bilateral nerve sparing RP. After RP I never felt sign of life in my penis and nothing worked for me. I used Cialis, Viagra, VED, penile injections TRIMIX, BIMIX with no success. I am 3 1/2+ years after RP.
I am also a Cancer survivor going on 11 years now. I had a very skilled surgeon as well but my outcome was nothing like Marks either. I even went to the expense of doing the Gainswave treatments to no avail as well. Using Viagra does help me but nothing is even as close to what I had before my surgery. I would do it all over again though to again be cancer free.

Hello Robert. I had my robotic prostatectomy almost 8 weeks ago and would be happy to share my experiences. Always open to speaking with men about this disease and recovery having gone through this myself. Hope all is well.
I only had one core out of the 12 that was cancerous with a Gleason score of seven so I opted for proton therapy which I will begin on September 13 for 28 sessions
The hydrogel spacer oar is now in place for further protection.
I was treated for prostate cancer at Moffitt Cancer Center using radiation therapy. No urinary problems or sexual problems.
Did the person doing the therapy say you could not have surgery if the condition came back and has your Cancer total gone now , Im due for surgery in couple of weeks

I did my prostatectomy 14 days ago and the catheter was removed on day 8. Since then I have incontinence issues with some days worse than other.
I can sleep through the night without any leakages and wake up and use the bathroom as normal but after that I start leaking and have wear my adult diaper for safety.
I know it's only 14 days but I feel as if I've made a lot of progress!

@@patrickdemeyer4668 no worries re English, I understand you. Your catheter should be removed soon. My PSA pre-op was also high too but my pathology results showed negative transfer to my lymph nodes.

One thing I didn't know before the DaVinci Robotic was the position you are placed into during surgery. You're tilted to almost a 45 degree angle to, head down, to get the abdominal Organs out of the way. I don't remember anything about the surgery. I had mine on Nov 10 of this year. Here's the big reveal. The pathology report after surgery showed no evidence of cancer. The MRI guided biopsy showed two very small areas with cancer. What gives? My urologist's initial comments, when I went to have the cathether out was, I have good news and bad news....You had no cancer but the prostate is out. No more worries. Right now I am dealing with the incontinence. Not so bad, we'll see. I wouldn't call it a walk in the park. I think the catheter was the worst part. Oh....I think something is a little shorter.

What did your biopsy reveal? And, regarding the 45 degree angle of the operating table, this is the first I have heard of such a thing.

I'm brand new at this, just was diagnosed within the past 30 days, and am very interested in what you say . Very low grade localized grade 1 with a Gleason score of 6, last PSA just above 9. The doctors really had to search hard for the cancer after an elevated and increasing PSA. Two standard transrectal ultrasound biopsies around a year apart found no cancer. After another increase in PSA - there's increasing consensus that that shouldn't be the definitive indicator - urologist called for a prostate MRI. That showed a lesion which lead to an MRI-guided biopsy last month, resulting in the diagnosis i mentioned.
My question to you is was the prostatectomy recommended after a definitive diagnosis from a positive biopsy, or some other test?
I did have an MRI guided biopsy and they found two of the twelve cores had cancer. One had 1% and the other 5%. He said it was not agressive. When I questioned the doctor, he said that there was cancer and sometimes the pathology done during the surgery may miss the exact location when they section the specimen. In answer to the 45 degree tilt - no damage. Logically, it makes sense to get let gravity help keeping the operating area unobstructed. I had the same Gleason score and my PSA was at 9.X. I was given a choice of radiation but after speaking with the radiation oncologist it was something that wasn't going to get it all out and I don't trust radiation. Besides I live 100 miles from the treatment center and he wanted me to go for 9 weeks, 5/days. Too much. My urologist said that radiation would leave scar tissue making surgery afterwards more difficult. I just decided, let's get this over with. I am 70, not getting younger. At this point, I am glad it's over and I certainly wouldn't do the watchful waiting. I realize this a long time in answering. Rarely check google mail.
Yup, mine too. Lost a half an inch, and for a Black guy who didn't hit the Big Branches on the way down the tree of life, I NEED my half inch back... Lol. I can laugh now, but I've read that it grows back to normal after a year or so. I stretch it out a could times a day when I go to pee, only a few seconds at a time.
The shrink is caused by the cutting seat of the seminal vesicles, the time that sends the sperm fluid thru the urethra. They cut off last if them and pull back in the urethra so we can pee, that's where the loss vibes from.

What about hormonal therapy that generally accompanies prostate surgery and its affect on libido?

Hey Mark, Thank you for having the courage to share this great informative video. I am diagnosed with Prostate cancer and await a bone scan and MRI to determine the appropriate course of action. My urologist wants me to have consults with a surgeon and radiation oncologist within his group. If I elect to have the surgery, I want to get a second opinion before proceeding. Have the surgery, get the cancer out of your body immediately. Just remember it’s a life changing experience and you will probably not be the same but life does go on and you will live a great life. Just your sex life is rather challenging. Sorry for being so straight forward but as men you need to hear the hard core truth. It’s a bitch

A bit of caution, not all will have a satisfactory outcome. Only 10% post robotic prostatectomy will regain pre-op function. It’s great you had the outcome that you did but perhaps best not to make a categorical statement either way. I’m a year post-op and I’m doing better than some, but I have a way to go.

Interesting video, I am sure this will help some guys quite a lot but as you stated, it depends on the skill of the surgeon. Guys you need to speak at length and in detail with your surgeon pre op. Its all about how many nerves are saved and left behind, the more the better, thats it in a nutshell, (excuse the pun) Three years now since my own radical surgery and I take 100mg to this day. Here in the UK you can get it free with prescription, I get four 100mg tablets a month and they help tremendously. I was also given a penis pump but wearing a ring was not comfortable for me on a personal level. There are other treatments on offer here in the UK. You will also find that you will need to change foreplay, sensitivity is not the same, in my case its not hammer and tongs as they say but a more gentle approach, gentle massage from your partner in other words. Im sure many men feel different in so many ways after surgery, we are all different, good luck to you all.

Biopsy revealed no cancer but, had BHP for 4 years now PSA the highest 5.3 for years. I'm crazy just getting my mind psych for possible cancer because my dad had his prostate removed at 65 y/o.
H B - Did your doctor find a "nodule" during the exam? I'm at that point so far, and I'll be doing my MRI and biopsy soon.

Hope everything works out for you my friend stay away from the radiation treatment . I knew a doctor who had radiation treatment it was the worst thing. Better off if it gets worse just have the whole prostate removed.

I used to take flomax Dr. gave me and then pro car & (Avodart) Finasteride (Proscar) also they use it for male baldness. I don’t use anything the side effects of too bad. i recently spoke to a surgeon about having the surgery and was told that patients generally were incontinent for about ten months after the surgery did u find this to be true
Had the bilateral nerve sparing robotic procedure 2 months ago. Have been on 5 mg tadalafil daily since the catheter came out. Still absolutely no hint of erectile function. Incontinence is improving very slowly however; one thing at a time, I guess.

My husband had his prostate removed a year ago. The meds such as viagra does not works at all. He had a high sex drive before. Nothing is working now.I’m ok . I’m glad he he is cancer free. I would recommend a 12 volt battery with heavy jumping cables. A lot of men that has had a prostatectomy don't ever again get an erection. I had mine three years ago and is dead as a doorknob. GONE!

A few men may be lucky enough to have sexual function, but in many, many cases they will not. Had the "nerve sparing" surgery in 2008 and have never had an erection since. Still have some minor incontinence to this day, but it's manageable. I am cancer-free with PSA of 0, and that's the important thing. That being said, I would be hesitant to recommend surgery to anyone. I believe there are better, less invasive treatments nowadays.

Your comment is giving false hope to about 80-90% of RP patients. I am glad it worked for you. I also read comments from other Dr S. Patients that are not so good like yours.
RP is terrible surgery and lives most patients non functional for the rest of the lives.
I had this surgery 13 months ago, nerve spared bilateral, but as of now, no sign of life down there despite aggressive penile rehabilitation. Cialis and Viagra did nothing even on max doses. VED never produced usable erection. TRIMIX gives erections in 7/10 attempts. Lost 2+ inches of length and lots of girth. Prior to RP never had problems with achieving and maintaining erections as long as wanted. Young and very active, non smoker, not overweight.
From my experience, RP should be last resort for young guys, not first one.
Mirko KLJAJIC I am sorry that you had a bad experience with your prostatectomy, however, it doesn’t serve others well to deliver a doom-and-gloom message about this experience. There are always many factors that will determine a patient’s outcome, and despite what you believe, the VAST MAJORITY of men who have surgery early, and with a competent surgeon, will recover very well. Don’t let a handful of bad experiences become the message that frightens others. If you have prostate cancer the prostate must be removed.

Prostate does not to be always removed. Do you always remove prostate with extraprostatic extention when Ca is not organ contained?
So whoever wants to preserve sexual functionality as long as possible should avoid surgery like a plaque. Even those 10-20% who claimed sexual function recovery are not always satisfied especially with more and more time passing by. I do not know how old you were at time of your surgery but I barely turned 50 and had excellent sexual function before surgery. Never before used Cialis, Viagra etc.
Now what? I am nonfunctional 100%. Who can be happy whit such outcome? Not even 77 year old guy.
I really wish I can talk about good recovery but I can’t.

His positive outcome is certainly not the norm. ED continues on for at least 50% of men even after 2 years. I am told I have a 50% chance of having an erection when on pills at 6-9 months.

This is an odd question but the thing that drove me crazy--during and after surgery I had to have a catheter taken in and out---It was the worst experience of my life The awful feeling as they insert it is awful Did other guys have this feeling as regards having a catheter inserted?
I have no memory of catheter being inserted. And taking it out was a smooth, non event that lasted all of 10 seconds.

@@angelrosario626 my surgery was performed by one of the best surgeon in Sydney. He told me that the procedure of nerve sparing robotic prostatectomy will have less sexual consequences. Erection May come in six months or in two years. Now more than two years have passed but nothing happened. I so depressed and disappointed.

The predictions of the urologist did not work out for me. Incontinent, ed and a destroyed orgasme. But cancer free for the time being. Two years now.

Coming up on my 1-year anniversary. Cancer-free and grateful at 61 years old.
The incontinence went away pretty quickly, heavy pads at first with some days requiring a mid-day change, to just the light pads, to not really needing them after about 4 months.
But still no erection. Daily Cialis - check - going on 10 months with that daily regimen. Tried the full-strength pill after about 6 months to see if I could achieve an erection, but only got headaches. Started penile rehabilitation with every-other-day vacuum pumping, but that has been mostly to maintain penis health by keeping the blood flowing into those tissues. Still no erections. Thankfully I can self-stimulate, at least, to orgasm.
My doctor is now saying that it may take up to 2 years to get back some sexual function and I'm trying to keep a positive outlook, recognizing that my worth as a man is not rooted in a hard-on.
Every single video about prostate removal talks about "sexual function" and "erectilie distinction". No one talks about ejaculations and orgasms. And all testimonials are a sakes pitch for a certain doctor or hospital. Can anyone tell me about orgams after prostate removal?
what were your PSA levels im talking to all the men my husbands PSA is 5.9 we dont know if he has cancer yet they just did his Biopsy today he is only 47 yrs old.
Rosalinda, I will give you a brief view of my prostate cancer. In October 2017 I was diagnosed as having metastatic prostate cancer via an MRI. One month later I had a biopsy done which indicated 17 out of 18 cores tested positive for prosate cancer. My PSA was 3.9. The moral of that is PSA may be an indicator of cancer but it is not completely definitive. Because my cancer had inductal properties, my urologist refused to remove the prostate. He referred my case to the Cleveland Clinc, who recommended hormones, chemo and radiation. I opted after much research and talking to prostate cancer patients, to go to the Dattoli Cancer Center in Sarasota, FL The Dattoli Center only treats prostate cancer patients and he does not believe in removing the prostate. Instead they do Dart radiation and seeding of the prostate. I highly, highly recommend any man with prostate cancer contact the center for a telephone consultation prior to making any decision on treatment.

@@rosalindalawson4669 I do not have a lot of money either. I drove 1500 miles to Sarasota for treament and fortunately I had the use of a relative's condo to stay while I was there. Dr. Dattoli does work with his patients when it comes to insurance and money. Remember your health is not reglated by the price tag. There are ways to help.
