Hello Dr Randy, thank you so much for your time and for your great work in these videos .. can you please fix the issue of the behaviour file in your website because it's showing the biochemistry file ! Thank you in advance.
For some reason I think Randy Neil writes the question for the step exam, I did my exam few weeks back and I’m came across a few questions he talks about.well done
Crazy how not many people are watching these videos. This is pure gold. Never knew that about aggregated vs phosphorylated Tau protein. You are a life savior Dr. Neil.
You're a ray of hope to many med students like me Dr Neil.. Such a helpful one, your videos helped me to dive into Qns and kinda dissect before actually picking up the ans.
Thank you Sangay... I am glad this has been helpful. Let me know what topics that you need help in and I can see what can be made to simplify things...
Randy Neil, MD Dr. Neil thank you for you’re help, it would be nice if you could make videos on Absolute risk reductio, attributable risk, number needed to harm and number needed to treat. I always wind up getting them wrong .
Showing us how to navigate the questions as well as breaking down their concepts in simple terms is no easy task and you do it incredibly. Thank you so much Dr. Neil!
WOW, you kinda blew my mind by how easy you make distinguishing between these disorders is, Thank you so much Doctor! :) I do have a small request about the effect of Vit. E on the nervous system "differentiation with Vit. B12, other than labs", since i saw multiple questions about it.
Hossam, Let me look to see what I have... The hardest part is trying to organize it into "meaningful chunks", but I will definitely look. Thank you for the feedback!
Love the videos Doc. Was wondering if you could simplify breast pathology as its a confusing subject? Appreciate you always and thank you for making this step 1 journey fairly simple.
IWATCHDEATH is the acronym for the common causes of Delirium. Might be easier to google an "image"... but important. Definitely a STEP 3 level concept. Thank you for the feedback.
Are you going to make any videos for Step 2? I will be taking that next year and I can tell that your questions are very high quality. They are not super tricky or complex unlike some test banks try to be, yet they adequately test important concepts that students are likely to overlook. If you were to release a book or something, I would buy it in a heartbeat.
There's a strong "overlap" with the information... I need to make sure I do a better job completing the TEP 1 material before going for Step2... In hindsight, I should have done a better job with describing the differences inside these questions... Step2 moves more towards "treatment"...but still references Step1 material imo.
Hello... I want to say that the the person, though might have hallucinations, still with agitated behaviour pattern so prominent makes it more with Pick Disease. Also, the patient doesn't have parkinsonian presentation
I was wondering if I could ask you a question-if an older person develops a delir after a major surgery for example- what is the outcome ? Would delir increase the mortality? Or make a mild cognitive impairment worse so it could lead to dementia? As you said a delir is something that happens FAST. but how long can a delir last? Can there be any permanent damage to the brain? Thank you !!!!
Great question... Older people susceptible to delirium. I am not sure what the data says about mortality. In terms of longevity, it seems to vary. The key is to get collateral about what their level of cognition and functioning was before the change. Hopefully, they can return to previous level, but sometimes they have a "new" baseline. Just my take of course.
@@RandyNeilMD I really appreciate you took the time to reply!! Thank you so much. So in the back of my head I always thought delir is something that comes fast and goes fast away. But I heard it could last weeks or in the worse case even months ? Is that true ? And besides some meds like haloperidol or supportive therapy - is there any other medication to treat a delir? Sry for so many more questions
If this is an error, please let me know so we can correct. When I look at my "old" FA, its in there as being different between the two (Alzheimers vs PICKS).
sir at 8:39 in FA it's hyperphosphorylated tau protein then how do we decide to choose aggregated tau protein ? PS. Thank you for our videos, your biostats ones have saved my life
Hi you teach awesome.thanks for such kind of videos.can you make videos on ethics for step one because in september there will be increase number of such questions.and please expalin pathway for old img who has graduated in 2011 what should he do for match?
Thanks for feedback. I have taught ETHICS on semester...so let me see what I can dig up and see if they posted any standards for the topic. Keep studying hard in the mean time...
@@knowledgegiver795 Of course... You will wake up 5 years from now...might as well make it count. It's the journey, the work you put in, that makes it fun...
Make sure to put the Dementia in a category with a characteristic of each Diagnosis. Hope this is helpful.

The "aggregated" is from FA. I needed something to differentiate between the Alzheimers and PICKS from a physical perspective...

Thank you Emanuel. Hyperphosphorylated = Alzheimers; Aggregated Tau = PICKS... Hope this is helpful.
Do you have a program for students??
