Potions activate the potion above them when they're drunk. As Frizix said, the two health potions were drunk, each one activating the stoneskin potion. Those activations provided more than enough armor for the stoneskin potion to be drunk for its 3rd activation. Hence why armor shot all the way up into the low hundreds the moment the health potions were drunk.
It sucks now vs how it was back in the days. Talking about the effect as the bag itself is pretty good. You can still put potions there but you should focus on positioning more rather than make sure the Potions are inside there.
For a while now they have been different. The old potion belt effect of chance to trigger above pots is gone. Now it's like the first pot in each belt gives you one random buff and if the belt has 4 pots, it cleanses debuffs. So, not particularly important.
1 potion cleanses a single debuff cleanse, 4 potions triggers multiple debuff cleanses. So once you get a single potion in the belt, you've maxed out its utility unless you're planning on getting all 4 potions in it. Since most people like double-stacking their potions (like what he did with the stoneskin above 2 health pots), you'll never hit that 4 potion mark.
Tusk Piercer doesn't consume Spikes when being shot, it consumes it on star weapon trigger, so whenever Falcon Blade goes off both the Tusk Piercers trigger, consuming 2 Spikes. Since Falcon Blade attacks twice, that doubles to 4 Spikes whenever Falcon Blade goes off.

Both health potions activated it at the same time, and that generated enough shield to instantly activate itself as well.
