Like of course it's still core outlast, hiding and running but now you can fight back by throwing bricks and other things. You do some pretty messed up shit though, the whole point is they're training you to become a sleeper agent through meticulously crafted trials. The trauma comes from playing psychosurgery (the hardest difficulty) sometimes it gets really intense, especially in the roguelike mode Escalation where you keep going, the levels getting harder with different, random modifiers (like no batteries or a bomb strapped to you for example) and if everyone dies your run ends. Sorry to yap I just LOVE OUTLAST RAHH
