It's a survivla horror rail shooter adventure game and the way some of the design decisions feel absurd and counterintuitive helps elevate the tone of the story. If it was just a generic 2005 FPS the story and style of the game would suffer.
The gameplay works in service of the story and it's themes so I think its fine, I've never found it to be a complicated or unintuitive game to play. Feels like more or less a streamlined silver case where you shoot dudes

I think Garcian knew going into the game that he would win. A blink and you miss it detail, if you look at the spin of the cylinder, you can actually track the bullet landed in the 7th chamber. Garcian, being a profession, probably also tracked the position of the bullet and knew that it he would win so long as Keane took the first shot.
I like how Keane's body language is actually apprehensive the moment he 'wins' He knows he's (essentially) lying about it being a six-chamber revolver as is customary, but it could also be two more things - he knows the kind of man Garcian is already and is unsure if Garcian will just kill him(he would've, and still could've kept the deal afterwards anyways) or the possibility that Garcian would just up and honor the agreement immediately.
I think you know who would've killed Keane, but Garcian's pretty much pacifistic outside of killing heaven smiles (which I struggle to call anything remotely human) But yeah I didn't even realize that, good eye!

Sometimes, like here, they intentionally disrupt the line of axis in order to make the scene look rather strange or offputting, you usually see it happen when in movies people are meeting a dangerous or unstable person, so you feel uneasy. Just one of those odd tricks of the trade in moviemaking.
I'm not an expert in cinematography, but I can't help but feel like it adds to the scene. Killer 7 is a jarring, batshit story, and I wouldn't be shocked, if breaking the 180 rule was a deliberate decision. Or, they had an amateur directing this. Ya got me.
