They probably explained this in patreon alredy but choso uses blood clot to harden his punches (since there is no fluid circulating its as solid as a rock),Thats why his hand got that red.
The thing about choso in this fight is that he is learning as he is fighting, we can see that in multiple times like when he goes wolveriene and then times his blood exploding when yuji blocks it to blind him.

Choso is strong in general, but having Cursed Technique: Blood Manipulation, which is a hereditary technique of one of the 3 clans of Jujutsu Sorcery, while Yuji was fighting with straight up martial arts and Cursed Energy Manipulation, simply gave him the advantage at long and medium range, while also having Red Scale Stack for close range combat. Blood Manipulation is incredibly diverse compared to Yuji’s current moveset.

At this point Yuji is like a scrub. He's semi first grade-ish level, which is impressive for someone who just started, but literally all of the main bad guys are special grade curses.

We are used to the protagonist getting power from somewhere in order to win, but in this anime the cliches disappear, at this point Itadori is not that strong, Choso was far superior and Sukuna will never help Itadori even if he is about to die.
