As a doctor myself, and judging just by watching from this video, The Sims 4 wins. Those physical exams and not to mention careful observation on the symptoms are the things that won me over. Those steps are important for diagnosis! It's also not a rabbit hole and we have an actual clinic. Some of the tools and equipment may be silly but that could be applied to previous sims as well. The big downside of Sims 1 careers is that once your sim reaches the top of a career they don’t get to stay in that career. The game decides that your sims needs to try something new and kicks them from that career to a different one. So in TS1 I never let them reach the top 😂
Considering the fact that your sim actually performs the tasks in Sims 4, I assume Get to Work eventually received some hotfixes(not surprised if from a modder either), because it was a buggy mess for a long time. Even more so combined with Get Famous. I remember patients wandering off everywhere instead of keeping to the waiting room, to you then not being able to interact with them because coworkers kept getting in the way. The receptionist didn't assist either and just talked on the phone. When you did manage to get a patient, you had to requeue your objectives as your own sim kept cancelling them and wandered off.
