    Remdesivir applied from day 16 of Covid infection. Isn't that normally regarded as too late?

    • @kazoz3520
      @kazoz3520 Год назад +6

      This patient was immunocompromised, not only had a chronic poliovirus infection, but also chronic SARS-CoV-2 infection (still positive 15 days after infection, at time hospitalised). It's not usual either to be treated for 12 days with Remdesvir, but by day 12 (now 27 days after infection), although still testing SARS-CoV-2 positive, the patient's RT-qPCR Ct had increased, suggesting a reduction in viral load.
      Plus, remember the original EUA for Remdesvir was for use with patients after hospitalisation, not as an antiviral preventative for severe disease. This was due to an early US study only testing Remdesvir for hospitalised covid-19 patients (finding a slight improvement). It took a long time for later research to be accepted that Remdesvir is best used early after infection diagnosis, as an antiviral to prevent severe disease and hospitalisations.

      I took notes when I first began listening to TWIV when it started. I'd Google everything even though this was a novel virus -- unknown! It helped as did reading of whatever my go-to medical sites presented (Mayo Clinic, Web M. D., UCLA & Cal Berkeley med sites, Johns Hopkins tracking/tracing dashboard, CDC) for information about spread/transmission and what this virus was and how it worked. I am not a med Dr, but I grew up with two scientists and an English teacher. So, facts are important and I have comorbid conditions (COPD) I manage in the Medi-Medi world where disease prevention is not priority. LSS: Have remained disease free. TG! Am vaccinated/boosted & still wear masks. Have gently encouraged faith in professionals in those around me. Which is very difficult to do in my senior living environment, but I try my best esp in view of "long Co-V ID 19" which is heck of gnarly to me! Know several folks who suffer from this, and I am in no way volunteering for that interference in my artistic pursuits. LoL

    Please clarify and correct me if I'm wrong. The inactivated poliovirus vaccine (IPV) contains the whole inactivated virus, including the capsid (outer protein shell) and the viral RNA genome. The virus is grown in large quantities in cell culture and then treated with chemicals such as formaldehyde to kill it while preserving the structure of the virus.
    Because the IPV vaccine contains the whole virus, it can induce a robust immune response and generate immunity. The inactivated virus cannot cause polio disease, as it is unable to replicate in human cells, but it can still stimulate the production of protective antibodies. So why IPV should not induce antibodies to the polymerase as Vincent says (31:40 to 31:46)?. Not sure if all genomic RNA of poliovirus is inactivated, if not, it is possible that even small quantities of viral RNA might be used by the host cells to synthesize viral proteins.
    By the way, the case report about the polio patient treated with remdensivir is great.

    • @roncarlin3209
      @roncarlin3209 Год назад +1

      Vincent explained that. The polymerase is not part of the virus, it is produced in the cell after translation.

    • @MicrobeTV
      @MicrobeTV  Год назад +5

      I understand what you are suggesting. If the inactivated virus particles were taken up into cells, it is not clear if the viral RNA could get out of the particle. And if RNA could get out, I'm not sure it would be in a state for translation, and if there would be enough polymerase to stimulate an immune response. Can't rule it out but we will revisit the question next week.

    • @MicrobeTV
      @MicrobeTV  Год назад +3

      I asked Amy Rosenfeld her opinion on this, and she said, the viral RNA is crosslinked by formalin treatment so it would not likely be translated in a cell.

      I found this part of the podcast telling. It is frustrating the number of times I’ve been treated by a doctor only to discover they got something wrong on the initial visit, typically a rushed appointment. Why do we not create a summary of the issues/symptoms and then an understanding that the doctor and the patient agree to, sign off on, before treatment?

You already have familiarity with The Reaper, but they did have more than 'one song' and I would like to suggest the following Blue Öyster Cult songs to expand your horizons....
    "Burn'n For You" - I'm sure you're familiar with this one as it became anthemic on radio in the 19070s
    "Veteran of the Psychic Wars" - part of the soundtrack to the adult themed and animated movie classic "Heavy Metal"
    "Astronomy" - an immortal BÖC classic not heard on the radio. Actually, the entirety of the "Secret Treaties" album is worth a listen
    "Flaming Telepaths" - another 70s classic not heard on the radio from "Secret Treaties"
    "This Ain't the Summer of Love" - a very straightforward BÖC Rock and Roll classic
    "Godzilla" - need one say more?

Those vaxxed up boosted up folks seem to be the main ones suddenly developing aoutoimmune lupus like conditions. That spike protein mrna goes all over the body when injected and it affects all organs, blood vessels, bone and muscle, triggering an immune response / self attack on cells in all these parts of the body.

    Hello! Science writer & scientific editor here. Re decoupling and feeling stupid: so stop cheaping out and put money aside to hire science writers! Put the budget line into your proposal. If you're not hiring salaried, estimate $70-90/hr; we have to pay self-employment taxes and fund our own benefits out of that as well, and it's professional work. Make sure you include enough hours to get the job done. Don't expect someone's going to read, disentangle, understand, and revise your important 20-page paper or 15-page narrative in 5 hours. The better your writing is, the faster it'll go -- and it's unlikely that "better" has to do with whether you're a native English speaker or not. Has more to do with your compositional skills and sense of narrative.

    This may be a bit cynical, but since most of the US population who would suffer from vaccine-induced polio are anti-vaxxers maybe the government decided it's their problem. That does ignore the very young who don't get mom's immunity, and the very old with diminished immunity. Seems like somebody made a callous cost/benefit analysis.

      I say that because most vaccinated people may get infected, but won't get paralyzed and probably won't even know they have polio. The real problems happen when the mostly benign polio spread hits a pocket of antivaxxers.

    Trypanophobia and breast milk vaccinations, a new breakthrough?
    When it was announced that the mRNA vaccine was being found in breast milk, I asked some of the lactating mommas in my area (who had recently been vaccinated), if I could plug in for a while, so that I could be vaccinated without being jabbed. For all of those who are anti-vaxxers because of their fear of needles, we could convert them to pro-vaxxers overnight.

      I was really bad with needles, but actually the corona jabs solved that for me. I was being open (saying "I'm not good at this") and the nurse would say "don't worry, we are" and now it's happened 3 times with me hardly noticing - and I'm not scared any more.
      I'm still not watching though.

      There are several studies that show trace levels of mRNA in breast milk, much too low to afford immunity. During breast feeding it is common for small tears to occur in the nipple resulting in contamination of blood in the milk. The concentration of mRNA was much higher in the blood and this is thought to be the source of mRNA in the milk.
      Perhaps, biting the compliant ladies in the neck would afford a more therapeutic dose.
