    Dr. Isaias Irgau, MD, FACS, FRCS (Ed), FASMBS is president and co-founder of the Christiana Institute of Advanced Surgery (CHRIAS).     Weight loss surgery, also known as bariatric surgery, has been recognized as a safe and effective treatment for morbid obesity and weight-related health conditions, such as high blood pressure, type 2 diabetes, and heart disease. However, bariatric surgery is not for everyone. You may qualify for bariatric surgery if:
    You are at least 18 years old.
    You are at least 100 pounds overweight or your Body Mass Index (BMI) is 40 or higher (or at least 35 if you have one or more weight-related health conditions).
    You have been overweight for at least 5 years.
    You have made serious weight loss attempts with other methods but only had short-term success.

    Obesity is a rising problem in the United States. According to the CDC, 93.3 million adults, roughly 40%, were affected by obesity in 2016. That’s up from 36% in 2008. Along the same lines, the counts of severe obesity rose from 5.7% to 7.7% from 2008 to 2016.
    Despite the prevalence of obesity in the U.S. and the numerous, severe health risks associated with being obese, obesity was not classified as a chronic medical condition until 2013, when the American Medical Association finally recognized obesity as a disease.
    In drafting this article, I consulted with Dr Isaias Irgau of the CHRIAS, who noted that “the 1991 NIH consensus on the indications for bariatric surgery excluded obese patients with a BMI 30-35. Since then, much knowledge has been accumulated implicating obesity with a plethora of dangerous illnesses. Bariatric surgery is much less invasive and much safer than it was 30 years ago. The risk-benefit ratio currently favors bariatric surgery for patients with BMI greater than 30.”
    Despite the classification as a disease and the risk-benefit ratio favoring these procedures, obese patients still have a difficult time getting the professional medical care that could make significant improvements in their health and well-being. Bariatric surgery is a safe and effective for obese patients to improve their health, but our current healthcare system makes obtaining such procedures a needlessly difficult and complicated task.

    Multidisciplinary Approach at ASC
    In the ASC bariatric program, we take a multidisciplinary approach to help our patients to go through the multiple steps. Although there are potentially six main medical clearances needed, with the help of patient’s designated care coordinator, going through these steps is extremely easy.
    Let’s examine these steps in a little more detail. These clearances are required:
    Nutrition education
    Depending on the insurance, this can be three to six consecutive months of visits to a nutritionist
    EGD test
    If this test returns negative, no follow-up is required.
    If this is positive (e.g., Barrett’s Esophagus or H Pylori), more follow-ups are necessary to resolve each of the issues.
    Psychological clearance
    The following steps may be required based upon the patients’ medical condition as determined by the surgeon:
    Cardio clearance
    The patient will undergo an EKG or Echocardiogram. If the results are acceptable, the clearance is given.
    If the cardiologist deems the patient’s heart health is too risky for surgery, the patient will need further visits to the cardiologist for a treatment plan.
    Pulmonary clearance
    If the home sleep test is negative for obstructive sleep apnea, there are no additional follow-ups.
    If this test is positive, the patient is referred to a pulmonologist for additional study and a two-week APAP/CPAP compliance.
    Medical clearance from primary care physician
    Any issues uncovered might require additional visits to a doctor or specialist.
    Patients will also need some blood work tests (such as A1C and TSH) before the surgery is scheduled. If these levels are within the acceptable range, no further follow-ups are needed. If these levels are elevated, then the patient needs to follow-up with their primary care physician, which could lead to seeing an endocrinologist.
