No idea man, I'm sorry. CS6 is from 2012 and I don't even own a license for the Creative Suite, so I can't download it. I'd highly suggest upgrading to CC tho, you're missing on a lot :)
Uh don't worry about that, I found that the object has stroke and maybe that's the reason because when I set the stroke to 0 all the problem solved. Love your tutorial!
Unfortunately there's no way to reduce the grain size, but you can make the grain layer bigger than it needs to be, rasterize it (Object > Rasterize) and then scale it down. Hope it helps! :)
I've seen so many ways of doing this, but your way is so much easier to follow than the rest.
Ugh, illy is just horrible with this. I sent all designs to Figma and it had the glass effect ready to go as a single layer.
Can't say I have much experience with Figma, but heard good things about it!
