Personally I paid about the same for my trd off-road premium. It does have the stock tires but everything else is the same on mine. Decided I wanted the off-road features if I was going to pay that much for one.
To answer your question, Toyota's philosophy is "if it ain't broke, don't fix it." Which makes sense if you look at the 4Runner, Tundra, and Sequoia. Toyota knows their customer base which a good amount of them keep their Toyota for a long time. You've probably seen some Toyota models from the 1990's and 2000's still on the road today. Yes, the 4Runner is old with the current design, but it works for people who are interested in buying it. Last, but not least, there will be a redesign for the 4Runner in 2022 as a 2023 model year. At least you get Toyota Safety Sense 2.0 as well as Apple Carplay and Android Auto currently.
Watch out because that 2022 model redesign for 2023, is most likely gonna be a 4 cylinder turbo. Once that happens, the 4Runner will never be the same. I’m making sure I get a 2021 TRD off-road premium.
How often would you want them to redesign the vehicle? Also, if the vehicle is already one of the best you could get, why would you want them to redesign it? I’ve got a 2021, and though it’s a long time away, I’m dreading my next vehicle because I know there will be nothing like the 5th gen 4Runner on the market ever again.

You may want to look at the 2021 product specs.. all 4Runners now have LED headlamps (ie. main headlamp) standard.
