Hi Matt, it likely would depending how you've rotated the UCS/setup the viewport. The Arrow basically follows the Y-axis. You could also adjust the North arrows rotation property to likely compensate for your UCS rotation if needed as well.

Would this ( north arrow) also work if you rotate the ucs of the drawing and not twist the viewport
