Not really, Malaysia is 3rd best English speaking country in SEA, despite English being seen as an "coloniser language" by certain community, you'll be surprised by most of them were quite boastful to have a good proficiency in English😅. Still, I won't say it is impossible, but it's not as difficult as some might think. If you have ever been there, you will see a lot of signage or memos in most commercial areas written in English. Most businesses accommodate English, and most businessmen speak English. The bureaucrats are more than capable of speaking English. I suggest you make your own research first, but so far, the language barrier should be the least of your concern.

I'm a Malay and just starting out in the business. I would say it depends on your products, some products could require more involvements with the more rural locals like Malays hence requiring technical support which preferably conducted in Malay language such as agriculture.
Nonetheless for the operation of importing and exporting I don't see it as a problem if your business ends at logistic side of the business. Hope that helps
