Should the front cover stick out from the wall a bit or should it be flat onto the wall?

front cover sits on top of the wall, otherwise you would have to chase the space around the inside to get it to fit flush, hope that helps?

HOw do you remove the front cover please?

flat head screwdriver, pops the plate off, or you you can bend the plastic to remove the cover, it is very easy
