"round start" simply refers to the position on screen at the start of a round. Doesn't necessarily have to be at the beginning of a match

Why do people always do the Kamehameha in the air rather then waiting for goku to smash them into the ground first? Wouldn't the damage be higher? Or does the scaling change then

That's exactly right. You get better damage by following up with an air Kamehameha over letting the kaio Ken finish and then using the Kamehameha follow up
