      Lol... She failed at the ABC part. Vecuronium bromide onset of action is immediate, it's used primarily for respiratory compliance with rarely used ventilator modes. Pressure control and rapid sequential ventilation. The goal is to prevent barotrauma(essentially exploding a lung) it is necessary to paralyze the client because both ventilation modes are viewed as very uncomfortable. This patient would have immediately lost consciousness, her extremities would become flaccid, and respirations would have ceased immediately. That is somewhat scary to think that she walked away without immediately noticing that. There is no grey area of Innocence that would excuse missing this critical assessment. Requardless of the drug mix up, administration of versed without a spo2 or telemetry monitor attached is a violation of conscious sedation protocols. This nurse should not have been practicing to begin with. It is truly scary to think there are nurses like this exist and are allowed in critical care environments.

      You should know the drug administering, every drug administered has a mechanism of action that we should all be aware of.

    The think is that this case is very sad, this can happen to any nurse no matter what kind of preparation have, the truth is that some people are grateful but others are ungrateful. Nurses have to much pressure from everyone patients, families, nurse supervisors, doctors ect. And it's extremely important to be organized and do nursing procedures not so quickly no matter what other people think. So anyway this is more for me as a way of learning to be more careful, effort not for what other people think just to be in peace. And it's important to be united as nurses because we have the power to do things right it just that sometimes we don't use it as it should be as saying in this video double check or triple check. Thanks for the video.

      It's a nurses responsibility to know the drug they will administer. This isn't a McDonald's drive through... It's people's lives! Every drug we administer has an action and sometimes an antidote to reverse an adverse action. To say this could say this could happen to any nurse implies risk. There is no risk when knowledge exist. Sorry, that attitude literally dimishes the role of a nurse and dumbs down our profession. All nurses have an elite profession .. to justify her negligence and dismiss it's severity by saying it could happen to anyone is laughable in the least . If you are truly a nurse you would know what a "sentinel event" is (ends in death) 99% of medication errors are described as a "near miss" (no residual harm). Iv over 30 years of nursing, I've never witnessed a sentinel event. Till this occured. Are you aware nursing is considered and expected to be the last line of defense against a medication error? That is a reality drilled into every nurse in college.

    Just FYI the patient was having a PET scan. She was discussing a swallow study on a different patient that she and her preceptee would be doing next.

      Yes, sorry I have misspoken. You’re correct. The court documents do state that the patient went down for a PET scan.

      @@SRNAPaul loved the vid otherwise. This whole situation is just a mess. Vanderbilt had 330 PAGES of remediations for CMS because of an entire culture there that helped to lead to this whole mess. Is she ultimately responsible for her actions? Yes. And she took responsibility. Vanderbilt has failed to do so.

    Moving forward how does this affect the field of Nursing? I feel this will push more nurses away from bedside and have a negative impact on the already short staffing we face in the future of Nursing. But, those are just my thoughts.

      am currently in second semester nursing school and have decided i will stay far far far from bedside because of this. As much as i love it through clinicals..... it is not worth my families life nor my own

      @@GreaterIsHeAmen I'm going to continue to practice as an RN in the USA. This was a negligent act preformed by the RN that resulted in a patient dying alone, paralyzed, unable to breath and died alone. It's truly was a horrible event. And I wouldn't tell you it's not safe to be a Nurse. Although, this is an example of how important to review the rights and understanding what these medications do before you push them. Stop, and think before giving medications.

      It's a sentinel event. It will affect every nurse... It should remind them this isn't a drive trough window. It life and death. It will certainly discriminate against new grads, they will no longer waltz into the intensive care immediately. It's a good thing. Not every nurse is equipped to work ICU. That discrimination was lost over the last 10 years. It's about time we return to age old standards of selection. Too many new grads working the unit . That alone is a scary thought.

    This is incredible for everyone who goes and trust their lives with nurses. They are literally train trained in that specific job sector to handle medication and administer them. If they are able to perform those duties 100% of the time then they will criminal charges. lucky she didn’t get prison time

    Bad employer. Vanderbilt is not a good company to work at. Better to keep away from this hospital. It doesn't care about its employees as well as its patients.

    It s like the patient s life meant nothing.Think of patient couldn't breath at all.What did she go through.Nurse didn't know difference between meds.She admitted she didn't remember adding water before.Yet she saw the skull and crossbones..Yet she still gave the med.She was not a safe nurse .She had Alot of writeups for med errors.

    Hospital didn't inject the shot that choked the patient to death .She gave the shot.The patient died not able to breath.How horrible.

    I know I'm in the minority disagreeing with so many of my fellow nurses, but no one but RaDonda was responsible for her fatal med error.     Good presentation, ironically you made a mistake in the beginning by saying that the patient went for a swallow study when it was for a pet scan. It is easy to make a mistake.

      Neglecting the standards of care when transporting an ICU patient was an unexcusable fuck up.

    Well we really can’t get mad at the hospital for covering it up because being in healthcare we are always taught to COVER YOUR OWN ASS and that is what they are doing

    Heartbreaking yes but she still killed an innocent women. The whole video you talked about the poor nurse what about the poor woman whom is dead. That lady was a wife, a mother, Grandmother the whole video you talked about the poor nurse! 🙄

      But you talk like she intentionally did it. It was a mistake that unfortunately had that outcome. We're all prone to make mistakes and will you get in jail for 1 mistake unintentionally?

She is in prison bc she did not do the bare minimum and read the damn label. Justice was served she is where she needs to be “in jail” RIP Charlene Murphy !!
