I'm having a weird problem with this. I'm on the XBLA (EU) version; this doesn't happen if I run it on emulator on my PC. If I use the buffalo star setup, I am misaligned; it consistently gives me the second picture on the right (wrong sub-pixel). I can still get the trick by aligning myself manually. Any idea why this could be happening?
I cannot confirm issues with the PAL version of XBLA to know if it is indeed different. I would guess you just need to adjust your timing of the throw. Going from emulator to XBLA is a huge change, and this is a trick of 1 pixel. Another thing to check is if you are moving forward slightly while standing up from the crouch. This seemed to be an issue for 2 or 3 people I taught this to. Just stand straight up after the crouch.
I was definitely moving forward slightly on a few attempts but I caught it early on and cut it out. It could definitely still be the throw timing, though; I'll keep working on it. In saying that, I spent over 30 minutes trying to get the setup to work and got it on my second try after just aligning myself manually. I suppose blind luck is always a factor, though. Thanks for the quick reply and I'll write back if I can confirm or deny that the setup doesn't work on the EU version.
Turbodog702 After quite a bit more practice, I've found that it is just the timing on the buffalo star. Seems quite different from the emulator. Still not 100% consistent but I'm getting it about half the time now.

Chris Wilson I'll try to talk to some other PAL runners and see if they have had similar issues.

Chris Wilson Yeah I talked to 3 runners with PAL version and they reported no issues. I wanted to talk to 2 more before I gave a response.
