There's actually a really good reason why good tattoo artists don't use local anesthetics, and it's the delayed reaction. I'm working on a bodysuit and I was talking to my artist about it, he said, basically, if we did a small spot, sure no big deal a few hours later. The problem is large scale pieces, where people would pay that premium for more comfort, your skin realizes you're entire back has been ripped to shreds in a minute. Your skin isn't going to react like it got 1,000 6inch lines all over you in a 6 hour window, your skin is going to think you just got hit by a fucking bus and it's going to react accordingly. Bruising, swelling, ink weep, again, there is a very good reason tattoo artists don't like the idea in their shops.
