Pancreatitis | Acute and Chronic Pancreatitis Nursing Lecture Symptoms, Treatment, Pathophysiology

  • Pancreatitis: acute pancreatitis and chronic pancreatitis nursing NCLEX review lecture on symptoms, treatment, pathophysiology, diet, and nursing interventions. Pancreatitis is inflammation of the pancreas that can lead to the digestion of the pancreas by its own enzymes and/or irreversible structural damage to the organ.
    There are two types of pancreatitis: acute pancreatitis and chronic pancreatitis. In acute pancreatitis, there is sudden inflammation of the pancreas due to something that has triggered the digestive enzymes to become activated inside the organ (high amylase and lipase levels will be found in the blood) and the pancreas starts to digest itself and swell. Two common causes of acute pancreatitis include: gallstones obstruction and heavy consumption of alcohol. Typically, acute pancreatitis is reversible if the patient is treated promptly.
    Chronic pancreatitis is chronic inflammation of the pancreas (can be caused by repeated episodes of acute pancreatitis but most common cause is years of alcohol abuse) that has led to irreversible damage to the structure of pancreas. There will be loss of the function of the endocrine and exocrine cells (acinar and islet of langerhans cells), damaged to the pancreatic duct etc.
    Symptoms of pancreatitis include: sudden, very painful mid-epigastric or left upper quadrant which can be felt in the back as well, fever, increased heart rate, decreased blood pressure, Cullen's sign, Grey-Turner's sign, hyperglycemia, dark-urine, jaundice, steatorrhea (oily/fatty stools).
    What is Cullen and Grey-Turner's Sign? Cullen's Sign is a bluish discoloration around the belly button, whereas Grey-Turner's Sign is bluish discoloration found on the flanks of the abdomen. They are due to retroperitoneal bleeding. Watch the video for an easy way to remember how to tell them apart.
    Nursing interventions for pancreatitis include: maintain NPO status, inserting/monitor NG tube, controlling pain, administering pancreatic enzymes and stomach acid reducers per MD order, assessing urine, hydration status, diet education etc.

    Hi Sarah! I just want to drop by to say thank you and that I’m really amazed on how you’ve come up with these review guides. I’m sure it took a lot of brain, time and hard work to make the outline and flow of each topic and this gives me the sense that you really love what you do and you have great passion for Nursing. You’re very smart and generous. You didn’t keep all of your knowledge to yourself. You are helping all nurses-to-be in understanding these complex concepts in the easiest possible way. On our own, these concepts are very difficult to understand because we either didn’t encounter them yet in the clinical setting or they were just discussed vaguely in nursing school and were never understood. Your flow is very spot on and the way you speak and discuss makes anyone watching your videos just glued on like a student attending a real class. I watch your videos on my phone, in our family TV, on any device that everyone at home just knows you! You are a very great teacher!     It's so interesting to know the depth of pancreatitis. I was diagnosed with acute pancreatitis 2 years ago & the pain, along with swelling, was a horrible feeling. Being on a liquid diet & morphine helped me get back to my normal self. I definitely changed my diet. I am gluten free & dairy free now, no alcohol, no fast food, no added sugars. It definitely traumatized me but it made me more aware on how to take care of myself way better. Thank you for your explanation & dedication to help us understand 💗💕 you are very appreciated!

    Sarah, thank you for all of your hard work and dedication in helping us become nurses. I struggled with the NCLEX. I needed to learn strategies and test taking skills. Your videos helped me. I pretended I was in a classroom & I took notes (the way you had them on your dry erase board). I did not have to study long at all and not every day. I passed my NCLEX with confidence. I knew I had passed. The test seemed so easy and I finished in a little over 2 hours. Thank you. Also, thank you to your family for sharing you with us.     This was very insightful. I was just discharged today after having acute pancreatitis. It suddenly came on last Thursday and I thought it was just indigestion. It wasn’t excruciating but more of a discomfort that lasted through the weekend. I developed a fever and a bad headache, which was worse than the abdominal pain I was feeling, so I saw a doctor on Monday and was diagnosed. Freaked out at first because I have a dear friend who almost died from it, but she also has other underlying medical conditions and she wasn’t taking care of herself properly. When they said it’s very treatable I was extremely relieved. I also started taking my health seriously back in June and decided to start working out for the first time because of a family history of heart disease and diabetes, and I knew I had high cholesterol. When they did the blood work and saw that my cholesterol numbers and triglycerides were healthy I was ecstatic. So that really helped rule out a cause. Exercise really paid off, and this video really helped me get a better understanding of everything, so thank you for posting this.

    I wish I had a teacher in nursing school who was willing to incorporate more biology into their teachings. I am someone who needs to understand why things work and not just told this IS how it works! These videos are awesome I can't thank you enough!!     Undoubtedly, you’re the best teacher of nursing! I love the way you teach, step by step, making sure that the information delivered is well understood, I admire your intelligence and knowledge etc. I’m using your videos to study for my NCLEX RN exam.     Thank you for this. It was eye opening. I was just diagnosed with pancreatitis today. Tomorrow, I have to have a covid19 test. Having lived with HIV for going on 12 years, I have no idea for why I have it. But, this pandemic and isolation, I see why people are drinking "more than they should" but, I thank you for this video. I didn't realize it could be SO bad. I will be educating myself more with your videos.

    Interesting presentation. I suffered for nearly ten years with non-ETOH Pancreatitis. Five hospitalizations with extreme pain. Finally subsequent to a cholisystectomy my episodes stopped. Thank God.

    You are just the best! Have my test tomorrow and your nursing interventions are right in line with what I need to know. Thanks specifically for explaining the relationship and rationale of using PPIs to inhibit gastric acid which decreases pancreatic enzymes, that connection I hadn't made before.

    Partly out of curiosity, partly owing to the fact I am no longer young and beginning to take more care of my health, partly because I am becoming a bit of a hypocondriac, worrying about every possible symptom, partly to keep senility at bay by challenging my mind, for all those reasons I have been taking an interest in medicine and biology recently, about which I knew zilch zero for the first sixty years of my life.
    It is wonderful how youtube can offer access to so much wisdom,wonderful that people are prepared to share it, and wonderful too is the complexity and genius of the organism. Either God or Nature is great indeed.

    I wish I would of found you 10yrs ago I was half way thru this and subscribed and watched it 3 times now . I have had to go to hospital 3 times for the pain 3 different cities and you could have saved me . You need to go around and teach some of these doctor's. I know there is a lot not known about this condition but wow you told me more about this than all the people that have treated me. But I don't blame them I forgive them . Thank you .

    Watching this video (for the third time 😊) sitting in my bed in very much pain due to the acute pancreatitis,I’ve been to the ER twice n only been giving pain n zofran meds. THANK HOU SARAH!! I’m getting my Lyft right now to hit the vitamin shoppe grab some pancreatic enzymes n go back to that dr and talk about these huge factors contributing to the pancreas... one very important I experienced an asthma attack n spasms with the morphine , so many different extra things that could take place !     Thank you so much for uploading this video! I am having tremendous pain in my upper abdomin and this video is helping me understand why that is. Man, I'm never going to drink alcohol ever again! The pain I'm experiencing, it's just not worth it.

    My acute pancreas started with intant extreme pain and vomiting litres of black sump oil looking juice . My throat linning and mouth skin dissolved. I spent weeks in hospital on a drip and nil by mouth. It took a week for them to realise what was wrong .I'm so thankful for our Australian health system

    In class when it was taught to put the PERT in applesauce, it wasn't explained very clearly as to why. BUT NOW IT ALL MAKES SENSE! They need the acid to activate! Thank you so much for that explanation! Also, these videos have helped improve my exam grades in class. I only wish I had started watching them sooner.

    I just want you to know how much I appreciate your videos. Something happened in my life and could not study for my finals like I should have so I watched your videos on Med surg topics at the last minute like for 2 days and got a 90 on my finals grade!! I plan to watch your videos on topics for next semester prior to lecture from now on in hopes of having better regular exam grades. Please keep the videos coming and keep up the great work!     Hello Sarah!! I have been following your courses since long. My NCLEX exam is also a bit nearer. And, nowadays I don't feel like going through the book as I have a habit of revising every bit from yur videos.     No alcohol use what so ever, no gall bladder. Reoccurring pancreatitis since 2016. Always mild clears in a couple days. This time much worse. Day 6. I did have Gastric Bypass 19 years ago. Nausea and back pain. Tender on left side.😢

Thank you so much for trying to reach out to younger nurses. I'm a LPN now for a couple of years but RN school is crushing me.     Yea, these videos are freaking awesome lol. It is great to see that you have a ton of knowledge in nursing and that you CAN TEACH IT without it being super boring and hard to understand. You explain very well and the tone of voice keeps me focused. Planning to apply to nursing school in 2018 and my goal is to watch as many of these videos and learn as much as I can before hand :D Thank you so much!!
