Just a suggestion. Maybe instead of using dental tools for the "extraction " use a nail pushy. The dental tool doesn't sound authentic as it's too sharp and scrapey. I think the nail pushy would be good and perhaps stick some blue tack in the ears you use with your microphone to enhance the realisticness of the sound
