It was just a mix between the kindred ult heal and the dragons immortality affecting its agro reset regen kicking in that raised the hp above what he thought it would be and missed smite. He was under no threat of dying at all, and no matter how zoned off the enemy is There is no professional jungler that would ever just “smite elder for hp” in a situation like that.

You should clip the way he gets his 8th stack in this game, that's the real big brain play and one only a kindred connoisseur would do. Nobody mentioned it on the cast but you can see it from the minimap, at 35:07 (game 2) you can see him tanking the big krug then standing still for about 15sec waiting for the mark to change.
AND it deals AOE 10% max health true damage. The item active alone is literally stronger than an ultimate ability, meanwhile engage sup items are trash
