YES on the extra shirt for yourself! My husband and I always kept an extra shirt for each of us in the diaper back when my son was a baby! It only took once of him spitting up on me when we weren't home and spending hours smelling like soured milk for me to realize to just pack an extra shirt :)

The car diaper stash is key! I would say the only other thing I typically have is a little cash or an emergency credit card in one of the zip pockets. I some times forget to put my wallet in the bag if I take it out and put it in my purse.
