    I was on Jardiance and then Farxiga and developed exactly what you described. In February 2022 I developed two open sores, one on my labia and one on my perineum. The surgical oncologist that I was sent to took me off the Farxiga. I had surgery to try to excise dead tissue and close the wounds July 2022. The sutures did not hold and she has decided to let the wounds heal on their own. I see her every 3 months and they are still open wounds. No hottubs, no pool time, and no intercourse in a year and I'm still fighting it. My nephrologist prescribed them and he was excited about their potential but I eat way under 20 carbs a day. I wish I had read the side effects when he prescribed them. Please heed Dr. Cywes WARNING!!!

    I was on Jardiance and Januvia last fall (both for Type 2) and I landed in the hospital in ICU with diabetic keto acidosis. I was told most people tolerate this drug combo quite well, but it almost put me in the morgue. I was quite fortunate that I didn't have a stroke, heart attack or organ failure as I guess it was touch & go for about 24 hrs. I used to take whatever was prescribed and wrongly assumed that it had to help if it was being given to me. Now I research all side effects and look to see what else I need to be aware of. Thank you for being such a great source of info!

    Taking a drug that helps one spill more glucose into the urine just strikes me as astoundingly perverse, whatever its benefits might be. I don’t think I could prescribe such a thing if I were a doctor…

    Since 8/19 after a quintuple bypass I’ve been Keto for 3 years and now carnivore for 8 months. Lost 195lbs, cured t2d, hbp, Ed, gout, sleep apnea and more. Was able to stop ALL 12 prescriptions that doctors/drug dealers had me on. Never had any heart related episodes. But they found I was blocked 80-90% in 5 arteries after 3 years of a vegan diet using a CAC score and a heart cath. My labs are A1C 5.4, HDL 60, triglycerides 60, ldl 105, glucose 102, BP averages 122/72. Cardiologist has pushed statins since bypass but I refuse.

    This subject is truly frightening. IMO, one had better get or become one's own medical advocate. Different scenario, but recently my GP told me to cut or break my Wellbutrin XL pills in half! I needed tapering off the drug and instead of writing me a prescription for a lower dosage, he advised breaking an extended release drug in half! Thank God, I knew better. My worst fear is being taken to the ER, in an emergency situation, and unable to monitor my own care.

    I've been on Farxiga 10 MG a day for 5-6 years. Type 2 diabetic, going to start ketogenic lifestyle. Thanks for the warning.. I'll have to get that changed.

    I completely agree with it not being a longterm option. I use it in practice for my new DM pts with hypertriglycidemia while getting them fat adapted using Therapeutic carbohydrate restriction with diligent monitoring (which i think is the key, people who cannot do it safely with good support from their doctor should NOT be one it). If they are overweight, having the dawn effect and haven't fat adapted I feel it's the lesser of evils. My ideal pt is on a glp1-agonist but it's cost prohibitive. Fortunately there is now a programme in Ireland where people who qualify can be reimbursed, I'm PUSHING hard! I had to fight SO hard for a pt who wanted it to stay on it because algorithm medicine dictated metformin, Fortunately he simply refused and signed an informed consent before he was 'permitted' Sigh

    Wow! I wish I had known about this before my T2 diabetic stepfather was prescribed Jardiance after his bypass surgery in March. He landed in the hospital 2 weeks after discharge in DKA. He spent an unnecessary 3 additional days in the hospital due to this adverse event. He has been low carb for some time, but we were not warned of the dangers of the drug for those with a low carb lifestyle. Thank you for this valuable information!

    Soon the Cardiologists will be promote taking pure cocaine. Honestly less harmful and no other medication drops weight faster. YES, later on followed by other consequences. Seen several actors and runaway models drop a lot of weight with this stuff, short term. However long term their life spirals out of control. Crazy world we live.     Thank you so much, my GP has been trying to push this medication on me. I am not diabetic and do low carb plus fasting. Female on Carvedilol and Entresto for DCM caused by a virus in 1996, ICD implant in 2021. GP is already upset that I refused Lipitor, because I do not have high cholesterol. I also carry genetics that can create side effects from Lipitor. I am suscribing.

    I was on Jardiance plus metformin, plus a lot of others. I felt dreadful, but my various consultants and primary carers were adamant it was all doing me good. I was already on fish / vegetarian / keto. I decided to go full carnivore a year ago, and removed the drugs stepwise. I lost weight, bp down to normal, mental clarity increased, more energy, and no pain killers. The only drug I take now is for blood clotting as I had double pulmonary thrombosis with no prior warning about 6 years ago.
    Feeling good.

    Did have ketoacidosis on Forxiga and spent the night in the hospital. Was working great until then. Was educated about the possibility that it could happen so was keeping an eye out for it. Felt like the keto flu from hell with extreme fatigue and a vague feeling that breathing was not right. Still had excellent o2 saturation. Glad I am Canadian because I don’t want to know how much it would cost in the states.

      I was in the ICU (USA) for 5 days last year. Had a clot removed from my lung, and then they put me under to shock my heart. I was in A fib.

    I do use SGLT2 inhibitors. I warn my patients that if they are ever sick they need to hold them because the anorexia that comes with acute illness can’t tip you over into DKA! I have seen it happen on a moderate carbohydrate diet in the setting of acute illness.

    Dear Rob, i am 64,on keto since 5 years and now being carnivore since a year . At 64 I go to the gym and do a lot of sport . At age 61 I have been declared with heart failure with a ejection fraction (EF) of 36.4 ( congenital caused by the myosin heavy chain 7 mutation). The Montreal heart institute took me in charge and I am taking daily : 25mg spironolactone ,5mg bisoprolol , 2x entresto 97mg sacubitril /103 valsartan and finally 5MG FORXIGA . Everyday I am checking my glucose levels 3.9 to max 5.7 and my ketones 0.4 to sometimes 1.9 with butter and C8 ) …I guess the dosage of 5MG of FORXIGA don’t do until now any harm ??

    I've had ketones of 80-160mg/dl, while doing a form of ketogenic diet, with Jardiance. 4 or 5 times, now. I think the highest was 120mg/dl. How high should it get for me to get concerned?

    Diabetes clinic in the UK knows I am doing keto but I was never warned about the problems associated with Forgia. I have had no guidance or monitouring by the NHS. I found out acid ketoisis can happen years ago.So what do I do? No chance of getting to a doctor or diabetics clinic. As usual left to figure it out myself.

I know less than 35 carbs per day I am in trouble. Last bloods done April 2022. I do not denigrate my GP, but I might as well not have one.

    A little off topic, but I tried Actos. Next day my morning gluconeogenisis went crazy. BG 180. I guess it doesnt work in ketosis. Saw nothing about it in the literature. I guess they dont test for that.

    Need to mention the compounded versions of these, the ones compounded to Metformin. Also, the risks can be managed by careful ketone monitoring. I prefer the mechanism of action of this drug to Metformin, and my doctor and I agreed on a plan on what to do if my ketones ever exceeded 7.5 mmol/L, which happened twice. I treated it like a low and ate 15g of quick acting carbs. It does require some care, and it's not right for everyone, but your title is overblown. For what it's worth, after two years of keto/carnivore, I'm about to phase out this drug, my last drug to manage type 2 diabetes. Interested to see what it does for managing my electrolyte intake.

    Dr Cywes, I started keto/carnivore 7 weeks ago. I stopped Jardiance . I was diagnosed about 32 yrs ago with 1.5 diabetic if that exists. I use my pump and a CMG. I do not take and bonus insulin now since starting the diet. I have not lost weight yet however lost inflammation throughout body. My insulin level went from 120 units per day down to around 40. Avge blood sugar 131 while I’m fasting. My Dr does not believe in this diet. I monitor my diet closely. I am still metformin. I will not have blood work done until July.

      @@robertcywes2966I’m in the UK, can’t find anyone with a good working knowledge of Low Carb. I’m on 10mg of Jardiance - told I can’t go on a GLP1 due to having had pancreatitis (70% pancreas removed), really don’t want to go on insulin. I’m Ketovore as best as I can be. Jardiance does help reduce my blood glucose levels and I’ve been ok on it so far….. unsure where to turn now 😢

    I was on it and went off due to recurrent UTI. After though there's still sugar in urine just less. Because my diet sucks. I have trouble switching diets because there's yummy sugar everywhere I look. Can't get rid of everything at home because family can't/won't do it. Withdrawal is pretty crazy too even if I don't have it for one day. ¯\_(ツ)_/¯

    This is maddening to me ! My husband had a stroke in November and has been put on 12 meds including Jardiance with metformin among 10 others. It’s madness…. I do a low carb higher protein and fat life and it works great , I can’t get him to do it hence the worsening of type 2 diabetes and having stroke. His doctors are morons that just push meds with not one word about diet , it’s insane to me but he only listens to them . If he is cutting back on carbs but still taking jardiance because his doctor tells him to , what are the odds of him having this issue . Very frustrating

    Just prescribed and have started taking jardiance 10mg, because my EF was 46% 🫤
