There was an interview with Damien few years ago expressing his regret over what happened with Lisa. When you put that into context, the song has a deeper meaning. Everyone experiences heart aches and Damien writes the anthem for the broken hearted and pained souls of this world, myself included.

This is absolutely about Lisa. They had a long history, and this song was also written a couple years ago (there's video of him playing early versions of it live in early 2012). I guess maybe it's about someone else, but this sounds like a relationship that was strung along for a really long time, plus Damien and Lisa didn't split publicly until '08 or so, it's entirely possible that their relationship lingered privately for a couple years after that.
The only person who doesn't know/ or care to know is Lisa herself. It's awful to see Damien like this, and although it's much too late now, I think this is really the only way for him to move on. Brutally honest, but he's not afraid of sharing the story with everyone. Forget Blower's Daughter, Cannonball, Volcano, etc...I think this is the most meaningful and important song he's ever written. This is Damien at his best - broken down and built back up all over again.
Based upon his interviews regarding Lisa Hannigan I have little doubt that this is about her. It's quite heartbreaking, and though she may find it annoying :) it is a beautiful song.
