    YAG capsulotomy is one of the most common eye surgical procedures. Approximately 1 in every 3 eyes that receive a lens implant will eventually need a YAG capsulotomy procedure.
    I share my own experience undergoing this procedure.

    Dr. this was super helpful and dissolved many questions I had. I'm just about a year out from cataract surgery and as you mentioned it's as if I'm looking through my original cataract again. Knowing there is light at the end of the tunnel ;) make s me feel much better about the process. Thanks so much for sharing!

    seems to me that ALL cataract patients should have the YAG treatment on their posterior capsule. At the place I went to, they had me sign a contract that said "No out of pocket costs within a year after premium IOL surgery" But my vision was so cloudy within months, that they did the YAG cupsulotomy - which improved my vision dramatically. However, they charged me $400 to have that done - even though there were not supposed to be any out of pocket costs within a year. They told me that it was not related to my cataract surgery - and was an additional procedure. They also said that 100% of cataract procedures result in opacification of the posterior capsule. I argued that if it occurs in 100% of the cataract procedures, then why is it being billed separately? The office manager was literally SHAKING because she knew I had her by the balls (if she had them). They also did not explain WHAT it entailed. I did not know until I watched this video! If not for Dr. Wong, there would be very little accurate information about cataract surgery, especially premium IOL cataract surgery.

    I will see my doctor in a week. He noticed early on after my cataract surgery that there were some cells beginning to grow, but at that time I barely could see a little blurring. He said I might need this procedure later on. Now, 9 weeks after surgery it's definitely blurred now. Looks very simple and quick. Thank you doctor!

      I'm guessing this describes the path I am on. The doctor noticed a "hint" of cell growth, three weeks after surgery. I noticed inconsistent minimal blurring or haziness. It is very confusing to me, as I already had/have a large floater in this eye and initially thought the blurriness was simply the floater. Also, the day after surgery, my eyesight was 20/20. It has dropped to 20/25.

Dr Wong explains the problem and treatment clearly, and then showing his own procedure was very illuminating (literatlly!) and also reassuring.     I just had the YAG about 30 mins ago and the only difference for me was I had complete clouding on my right lense and a 'cup' gizmo was put on my eyeball before the laser zapping. Completely painless and gobsmackingly quick. I'm looking forward to clear vision again, like it was after my cataract surgeries in 2021.👁👁 😁

      Yes I had this small "cup" device placed on my rt eye to prevent the eyelid from closing.

    I got my yag laser treatment five days ago on my left eye. My eye was so bad they couldn’t see the back of the eye on the scan before hand. I couldn’t see any letters on the eye chart so I had become legally blind in that eye. Would of been a 4 month wait if I didn’t change specialist. Only had to wait a month. New specialist was surprised how bad and quickly the scar tissue progressed and that I couldn’t wait 4 months. Took about 4 minutes to do. I can see clearly again now. Didn’t hurt just felt like onion near my eye towards the end. Had headaches down the same side I got done for three days and sun light sensitive but was well worth it.

      Just had mine done yesterday but vision hasn't improved 100%. Need to use drops. I also have headache on the side I had the yag. I pray my vision will improve

    This was a great video! I just had this procedure done yesterday (3/25/2022) and it went off without a hitch. Just a few hours after the procedure, my vision was fully restored to where it was post cataract surgery. Just a couple of small floaters, which I assume will go away eventually. It's amazing what they can do nowadays.

    Had it done today. No big deal. Took maybe 10 minutes. Have some floaters, but I had them already since gel in the back of the eye broke loose years ago. Main thing sight in that eye back to normal.

    Same here torn retina surgery. Three years ago, now double cataract only a week out already foggy and many new floaters. Also both eyes had scalpel astigmatism correction at same time.
    Will need yag also ugghh!!!
    My concerns are how does a laser not ruin the prescription of the new lenses?
    My one pupil has stayed constricted for 5 days now , causing eye pain and a very dark view!
    Very concerned about the total outcome.

    Excellent informative video. I just had cataract surgery a few hours ago in the left eye (Alcon Viviti IOL). Vision in the left eye was immediately opaque, worse than the original cataract lens that was removed. What causes the opacity during or immediately after the surgery? My previous cataract surgery in the right eye 14 years earlier didnt have this problem.

    I really liked seeing the procedure "live" ... many ppl say something is easy, but I like to see for myself... i liked that you can blink freely... I was worried I would mess something up if i blinked.... I had both my cataracts done this past june... my left has folded over and has a 'streak" (?) in the middle that i causing more glare than the other eye. ( they are multis so they have glare) ... I have a super high deductible so I want to get it fixed this year ... but on the other hand I am not happy with the lens.. 2nd opinion said it was too close to consider an exchange - just do the Yag and maybe get lasik later ( or glasses) to fix the remaining vision issues. ( he said there is no gaurantee that doing an exchange would be any better as the capsule and opening the eye again can cause their own issues) (sigh)

      @@desa1149 I had a gradual clouding of the new lens after cataract removal. I suspect this is a common experience. I plan on getting a visual acuity test again for near and far vision. I think my visual acuity is not the same.

    Hi greetings from the UK & thanks for this Vid - very informative.
    I had uncomplicated cataract surgery just under 1 year ago - my eyesight since has been excellent & I only need reading glasses now. I went for an eye exam yesterday & the optician said I had early signs of PCO & that he would monitor it over the next couple of years, I have no symptoms. He said the procedure to rectify it was straightforward & would not take very long. Is it certain that this condition will get worse or can it stabilise?

      I do not use a lens on the eye to perform YAG capsulotomy.

    Had my first eye done three years ago and had to get second eye done a month ago. Got a blurry spot that sweeps across my vision when i look from side to side..hoping that will go away 🙏🏻 Eye that was done three years ago seems to have a hazy spot again when light comes from the right hand side (right eye) is that normal or can yag need a “top up” can bits be missed?     Thank you Dr. Wong for the enlighting video, I want to make you a question, I had this double surgery where the doctor made a Ahmed valve implantation to release high IOP due to closed angle glaucoma and at the same time, the cataract and IOL implantation surgery (in the same eye)…, however, the next days (actually until the present) my vision remained cloudy… it was not that it became cloudy progressively over time…, since day 1 it was cloudy… mi vision never cleared up, I know I need to see another doctor but, do you think YAG laser would clear up my vision? And why it didn’t get clear after the surgery? Was it a malpractice? Thank you…

    Hi Dr. Wong, my mom had her cataract surgery in April, 2021, and she's already developed POC on her left eye. She said her vision is very good, has absolutely no problem. Should she still receive Yag laser treatment? (yag casulotomy)

    I just had this done last week as I had a secondary cataract which was the result of retina detachment surgery. I can say it doesn't hurt at all and is literally 5 minutes of your time. And you will see the positive result about half hour after.

    Thanks Dr Wong! I´ll have this procedure done on my left eye in two days. I came across your video trying to understand what the risks and ops of the procedure are. Your explanation is excellent, full of details and answering all of my doubts.
    I had cataract surgery 13 years ago and now it feels like cataracts are back. I can´t see the golf ball when I hit it. I see all foggy, specially in bright sunny days.     Hi Dr Wong! As always very didactic in your presentations. As an ophthalmologist, I would like to know what's the limit of energy we can worry about with retinal and macula complications when we perform a capsulotomy with yag laser. Some patients require more energy in millijoules than others. Thank you for your attention

      For YAG capsulotomy, my usual settings: 5.0 mJ for 30 pulses.

    Thank you for this information about the POC. I just had my left eye cataract surgery a week ago and today I found out that this eye has POC and they said it will be done in about three months. Don’t think I have this issue with my right eye so far. Really felt disappointed with this, but now I’m seeing how common this issue is.

    I had my cataract surgery in 2019, and had to go in for the YAG capsulotomy for my right eye today, and it was like you said... Painless and quick. My vision in that eye was very blurry before the procedure, and it's markedly better even though I am experiencing haze from my eye being dilated. I also feel a little discomfort like "dry eye conditions", and since I'm also a glaucoma patient, I am on 4 drops of Prednisolone for the next 7 days along with my normal 2 x dose per day of Brimonidine.
    Thank you for explaining this procedure in such simple terms. How often does this have to be repeated? I will be needing my left eye done at some point as it also has very mild symptoms that are just starting to show.

      They're essentially cutting a hole in your capsule and it can't grow back, so the capsulotomy is good for life :)

    Very happy with the results.
    My question, is the cloudiness of PCO there all the time or does it come and go?
    I sometimes get it in my right eye but it doesnt worry me very much.

      PCO stays permanent. It is due to proliferation of some cells behind the capsules

    What if after the YAG capsulotomy for PCO, one develops anteriior capsule opacitication? Can the same YAG capsulotomy be done to the front and will that cause the vitreous humor to leak to the front or the aqueous humor leak into the vitreous humor? Will this reultant mixing of the humors be detrimental to the eye?

    Do the yag yag in both 👀 but have to wait 3 months for next check. If you do the yag to early after surgery then your risking movement of your lenses.

    Great video....but if you have this procedure done, you can never put a different lens in your eye as there is nothing to hold it in place. I am supposed to have this done in October but am no happy with the lens I had installed so not sure what to do at this point. Maybe remove the lens and start fresh and see if that gets rid of the blurred double vision, I never had even with the cataract.

      It is possible to have intraocular lens exchange after yag capsulotomy.

    I had catoract surgery done 4 years ago when i had health insurance, now they tell me i need this procedure on both eyes, my left eye i cant see clearly at all but now i have no heath insurance, as a ballpark, what is this going to cost if i have to pay for it

    Excellent topic. I need a YAG. I am worried about IOL pitting and about people complaining that they see X in the glare. Do you prefer circular or cross YAG?

    Wooow, thank you doctor for shedding light! I am a doctor too (pedia), and I believe I've PCO symptoms now (1 year post cataract surgery). Also Noticed much floaters in the same eye as well. Are floaters a common complication of the cataract surgery I had or just age-related? Will definitely see my eye doctor. Thank you again. This is a very interesting vid.

    I had two successful cataract operations on August of 2022,and in February 2023. I have now developed the difficulties in vision caused be the capsule effect. This has been confirmed by my Optician. The operations were carried out under the provisions of our National health Service ,after rather a long wait. My optician has asked for urgent attention for the laser surgery but there may be a further delay..I am experiencing reduced clarity on the first eye and the second seems to be developing similarly. Awkward for driving and computer work.So ,grin and bear it I suppose.......

    Six months post-surgery, I'm starting to get some posterior opacification and I'm concerned after reading that newer lenses such as the RxSight LAL are more prone to pitting from the YAG capsulotomy procedure. Dr. Wong, can you make a video on how to find a qualified doctor in our area to perform this procedure with LALs? So, we can minimize our chances of lens damage.

    Hello, I am so sorry that I had YAG to my right eye, went blurry 4 day after cataract surgery, I now have a big grayish cloud like flash that swipes across my eye it is terrible when driving. My opthomologist sent me to a Retinol doctor and I have Posterior Vitreous Detachment and he claimes that is why the blurry vision and the what he calls a floater. I am so disappointed.

    I had yag. It didn’t clear the cloudiness. Years later after they told me it should improve it didn’t. I’m told my eye looks perfectly normal yet I can’t see so clearly something was done wrong but my doctor insist “everything looks good”. Yet I now have strabismus because I can’t see out of my eye, it’s so cloudy/blurry my brain decides it’s not even worth using

    Dr Wong, i see that there wasn't any small device put over your eye. I recently did the YAG procedure on my right eye at Vista Malaysia. The doctor put a small device over the eye that will stop my eyelid from closing when it blinks. What is the name of this small device? It's true YAG is totally painless.

    I had cataract surgery on Sept 16, it's been 4 days and I'm just now seeing 2 tiny black dots floating around. Should I tell my Dr or just learn to live with it? I also have flashes in the corners of my eye.

    Thanks for the helpful video. I'll need to have this procedure done next month after a consultation visit first. My question is whether I need to have my wife drive me to the ophthalmologist or can I drive myself back home after the procedure.

    Had this done on May 28th 2024, the Surgeon has to do 29 clicks to get it clear is that par for the course? I am seeing much better in my left eye now, had the implant done 3 years ago. My right eye has my born with lens. Didnt need that one done. Thanks for this video.

    I am having double vision. I only have vision in one eye. I had cataract surgery in this eye. Can this capsule opacification cause this double vision?

    Im planning to switch IOL in the future... Should I go for Yag now? Moderate signs of blurring is starting.

      not an expert, but i believe if you YAG its either impossible to switch IOL or very hard (dangerous)

    I had catsract but now i need yag laser. I had rd before. Will the laser damage retina? Is there an alternative surgical procedure? What is some laser does hit retina? 👁️
    My retina had lots of lasers during rd. I must see clearly otherwise i can’t function.

      Same thing happened to me the surgeon did lousy job.
      I finally had to go for yag last week.

      @@ricpowers1475 I went for the YAG procedure after a year. my eye sight improved tremendously, however I see a lot of floaters. I spoke to the surgeon today he stated they will eventually disappear on their own or my immune system will eliminate them. As long as the numbers of floaters are not increasing, there is nothing to worry about.

      @@nadiaj8002 Describe floaters...do they obstruct vision? Oddly enough my insurance covered 100% of the cataract surgery but does not cover ANY of the yag surgery.

    I see good after cataract surgery and I see very clear now no blurry at all but my doctor recommended to do YAG laser which is I don’t see blurry do I have to do the Yag Thanks

    Is there a hole made by the laser in the Posterior Capsule when undergoing a YAG procedure? If so, is there now an opening to the fluid in the Vitreous Cavity that the fluid can enter the Posterior Capsule and would now come in contact with the back of the lense?

    Can this happen just a month after having cataracts removed? I’m seeing my eye surgeon Thursday because my right eye got cloudy, I’ve been dealing with some severe dry eye since having them removed so my vision is still pretty blurry but now the right eye got cloudy and that’s scareing me..

    Hey guys im 28
    I was diagnosed with cataracts in both eyes last yr.
    I replaced both lens so far. However the right lens is now frosted just 9 mths after. As im a young one can i still live life as normal after YAG?
    E.g run lift weights...get the occasional hit or dust in the eye?

    Why have I been plagued with starbursts after having this? It’s been two weeks and I’m not getting any answers.
    I have to see a retinal specialist and get a retinal flush to see if they can identify any particles in my eye that could be causing the problem.

    Does this procedure actually make a hole in the capsular bag, or does it just clean the bag? I ask because I wonder if it would complicate a future IOL exchange.

    I had cataract surgery on both eyes in 2020. On this day (05/24/23) I am had PCO on my left eye. Can’t wait to see better in a few days. I just wished that I didn’t need to continue to wear glasses.

    Thank you so much for these highly informative videos. The way you describe things I am experiencing is reassuring. At least I don't feel so alone and crazy because I'm having these problems.
    I just saw some videos about how bromelain supplements and/or eating pineapple (which contains bromelain) can dissolve floaters. Have you ever heard of this, and if so, what has been your patients' results?
    Please do more videos on positive dysphotopsias, Crystalens implants, monovision vs. blended vision, and whether things really improve with healing/time, or whether we just learn to tolerate things we might not otherwise tolerate.

    What if my post cataract vision is not smudgy or foggy, but more like simple bad vision? I've been diagnosed with PCO and already have moderate floaters, so am quite concerned about getting more. Also, it SEEMS like my vision should be fixed with the right lens prescription but that has been elusive ever since my cataract surgery...never has been clear enough to read street signs unless on top of them (too close to turn, for example). Any thoughts? Thanks

    Would be nice if there was some sort of laser surgery for cataracts, and then vacuum out all the debris/floaters it leaves behind. I am just starting to develop cataracts in my left eye and I cant' stand it. Even with corrective lenses I get a some blurriness/ subtle double vision or whatever it is.

    My view is clear in ther left up corner, the rest is smuged like you explain. I have my treatment tomorrow. Looking forward to a clear view again. I was getting very nervous so i looked for more info to keep my calm. This really helped! Thanks.
