Hey Tiffany, Im a medical student and I wanted to help you understand the staging of colon cancer. What you have showed us- T4b N1c M0 is your TNM staging. It describes the size of the tumour (T), if the lymph nodes are affected (N) and if you have metastases in any other organs (M, 0 meaning that you have none). This TNM classification determines in what stage of cancer you are and in this case you are in stage III. Stage IV is for the patients that have distant metastases f.e. in their lungs. You can type "colon cancer staging" and read how different TNM means different stages. So the "T4" doesn't mean you are in the stage IV of colon cancer.
I'm not sure if you were genuinely asking if the letter was good news or if you just wanted some clarification, but if you're still unclear (you said you've spoken to your team so if they've already explained everything you can just ignore this! ), I can explain what all those confusing letters mean. T4B indicates that the tumor has penetrated the colon's wall and is either attached to or is moving in the direction of other surrounding organs or tissues. N1C indicates that the cancer has not yet reached the lymph nodes themselves but has spread to nearby fat or tissue. M0 indicates there's no detection of spread to distant organs. You've said that you value when we say things honestly and/or bluntly, but I can also understand that this comment could be overwhelming. Feel free to delete my comment if necessary, Tiff. @@sheouttapocket6637 No! the other markers are more important. Specifically, the (M) marker is the most important in this instance, which stands for metastasis. I'm not her oncologist and there are obviously things we aren't aware of but M0 indicates it hasn't spread to distant organs. The diagnosis markers she showed look like Stage 3C to me. M1A/M1B/M1C would be a Stage 4 indicator. I think Tiffany said they aren't worried about the spot in her lung but maybe the doctor she saw mistook that as Metastasis which is why she mentioned Stage 4.
@@sheouttapocket6637 no it is actually stage 3... in Canada it would just be called stage 3 a, b, or c depending on where the tumour has grown and how many lymph nodes are involved. I think the number 4 confuses people so many people think it's stage 4 but it's not.
You sound like you understand the situation better than Tiffany does. It's extraordinary that her oncologist are not explaining these things to her in such a simple way. Xxx
Tiffany, I have been following your journey, and as a retired nurse I must admit the whole situation does seem a bit confusing. I urge you to be more assertive during appointments, and not leave until you have the answers you need. Don’t worry about taking up your doctor’s time; it’s what he’s paid to do. I find it helpful to make a question list and go through it systematically during an appointment. I would certainly want the cancer stage explained fully and clearly, and the treatment options going forward outlined. Failing this I would seek a second opinion. There are lots of excellent oncologists out there. Whatever you decide my thoughts and best wishes to you.
I agree, I'm not a nurse, but I can't understand why it seems that information is so lacking or badly explained. A second opinion would be good, I think.
@Laney 26 this is unacceptable she didn’t even know her stage. How is that possible ? Isn’t there one assigned oncologist for her ?? If there is a team they also should know her file. I am not a doctor but this is what I would like it happen in a real World.

As a retired nurse I am very thankful that you are sharing this process that is very personal. I am thankful for your honesty, seeing the support from family, friends and hospital staff. The confusion that happens affects you more than most people understand. I am proud of you for fighting hard, completing your course of treatment and going on to the next stage. I'm not questioning you however I am questioning the cold shoulder the surgeon is giving you. He is still sick?? It's been quite a while. And he can call you, video chat, text?? In this day and age there are so many ways to communicate I find it hard to believe that it is impossible for him to communicate with you or any of his patients. I work in a hospital and a lot of the Drs I work with would at the very least call even if they were sickly.
My docter had Corona and was ill for six weeks, he wrote e mails to me and explained on the phone my diagnosis. My illness was a twisted ancle and I need a XRay, also nothing bad. I don't understand that the docter can not communicate with Tiff. when she has so a bad illness. Sorry about my englisch
When using TNM staging, T4 means locally advanced; N1c means cancer is present in surrounding fatty tissue; M0 means no distant metastasis (spread). It means you are not stage 4. However, it still means that cancer is present. For your own peace of mind and in order not to waste time, please get a second opinion. It honestly won’t hurt. Take care of yourself, stay positive (as always) and we are rooting for you.
@@Laura-gd4ku don’t know how old that information (T4bN1cM0) is. Recent scans may show NED (no evidence of disease) but it’s always good to check to confirm this.
You may use a different coding in the UK. In the US, size is captured in staging, while grading (the T system) speaks to how different the cancer cells are from normal cells when viewed under a microscope, with T1 for example being the least different and hence less aggressive than higher grades. It may also be that different cancers use different explanations for the grades.
Tiffany, I really don’t think that people don’t believe you. It’s really hard for some people to believe that anyone with Stage 4 cancer could be doing so well. The fact is that cancer treatment has made tremendous progress over the years and cure rates are very good. You’re doing amazing! 😊❤
It’s also possible that it was a misdiagnosis by the doctors. There were so many mistakes made in the beginning of her journey, that I wouldn’t be surprised if they got the staging wrong too. No matter anyway. The important part is that she is heeling.
Tiffany the cancer staging systems are complicated and hard to explain. It sounds like you are overall stage 3 but the tumour stage is T4b, the nodal stage is N1c and metastatic stage is M0 so no spread out the pelvis. You can look up TNM staging for colorectal cancer and you should be able find a table with each stage listed. T stage 4b means tumour has grown through the wall of the colon. N stage 1c means tumour is attached to nearby organs eg uterus, vagina. Staging is very confusing and there are different systems. Question to ask is whether your new TNM staging is pathological, based on recent biopsies or is it based on PET/CT/MRI scans. If tumour is still stage T4b then there is still cancer evident. Some newish doctors or registrars may not be familiar yet with TNM staging and will find it hard to explain. I am a cancer research nurse so very familiar with it.
Also, it is confusing because in the subject line of letters they will usually always write your initial staging at diagnosis so you keep seeing that. It was hard to tell but I think that is what you showed not your new staging after biopsies and chemo. After you have your next scans ask 2 questions 1) what is my new TNM stage? and 2) what is my new overall stage of cancer?
so maybe what tiff is seeing in the letter is her early Diagnosis and not her new TNM stage deb? That would be great news,her new TNM stage is prob much better,I like the sound of that,hope tiff sees your comment deb thanks for explaining it to us xxx

I think it does matter alot whether its stage 3 or 4. It seems crazy that our best cancer hospital in the UK is so vague. Let's face it, if one in two is expected to have cancer at some stage then it's very worrying 🙈 . I'm wondering why more biopsies and why more scans so soon after the others - without really understanding in detail the last ones 🤷♀️ it's very distressing for you

Hi Tif… I really think you need to get clear information, I feel for you, for your family, I wish I could help you get around all the info. It doesn’t make sense that the surgeon is not available, also at the same time he told you to wait for surgery? Something is off.. and don’t get me wrong , you could be in a very stable situation right now and they don’t want to change that with surgery, but certainly there is missing info.. as a health practitioner that has been watching your videos and I still don’t get the whole picture.. staging is not the question for me right now, is what is happening with that mass that is/was there ? You have to expect comments both positive and negative when you put your life on social media. From what I see most comments are supportive and caring. Tiffany could shut down you tube for awhile, take a break, stop and smell the roses, as the saying goes, enjoy your loved ones, meet with your cancer team and settle your mind. Hi Tiffany - It's taken me a lifetime to learn this lesson and I want to share it with you. People who don't believe you are being abusive and you can't convince abusive people to treat you well. One more time, YOU CAN'T CONVINCE DYSFUNCTIONAL PEOPLE TO TREAT YOU WELL. It doesn't matter how much you explain yourself to an abuser, they will not change. They are abusive because of themselves, not you, so your behavior will not get them to stop. You can not convince them to feel empathy for you.Please save your precious life energy and do the only healthy thing when it comes to abusers and go no-contact. Delete, block, ignore. Save your energy and time for the loving voices in your life.
Tiff FYI - no shouting - I promise 💐❣️ - T4b N1c MO = T4 is split into 2 stages, T4a and T4b: T4a means the tumour has grown through the outer lining of the bowel wall and has spread into the tissue layer (peritoneum) covering the organs in the tummy (abdomen) T4b means the tumour has grown through the bowel wall into nearby organs.
Hi Tiffany, I think why there’s some questions regarding your diagnosis is because of your chemo journey and then your biopsy showing no cancer. It does seem very incredulous that you would go from TB4 to clear biopsies. The diagnoses you showed us with TB4 was for your right lung but wasn’t it determined that you didn’t have cancer in your lung after all? I think what people are asking for is your diagnoses for your colon cancer… I may be confused about this but I too am curious as to what your diagnosis was for your colon cancer. I am so happy for you if you are clear of cancer of course but it does seem a little confusing as to how you went from TB4 to clear biopsies. It’s not because we don’t believe you I think it’s because we’re a little bit confused. Can you show us your diagnosis for your colon cancer? I think this would clear up any doubts in anyone’s minds. I’ve been doing a bit of research myself on patients with your type of cancer at the stage that your cancer is at and apparently the wait and see approach is something fairly recent but it’s becoming more and more common. It tends to be done in cases where they feel that the cancer may no longer be operable. From what I’ve been reading, particularly after the level of testing and exploratory surgery that you’ve had, they want to give your body time to heal and also give your mind and emotions a chance to adjust. This is probably why they recommended therapy for you. I have never doubted for a minute that you have what you say you have, and I know there’s lots of different options in how doctors approach your type of cancer. I just feel strongly that you need to focus on what your reality is today and take advantage of this opportunity to get your mind in the right place to deal with all possible courses of action. Did anyone else feel scared when Tiffany said the woman swerved all their questions? What on earth?! It's YOUR body and you deserve to know what's happening to you! It should be illegal for these people to not to give the full information. Why are they holding back? I feel like they're scared to tell Tiffany the truth about what's really happening so they're stalling and avoiding questions. So unfair and disgusting. I'm so sorry this is happening. I pray that YOUR SURGEON calls you soon and tells you everything you need to know. Complete transparency (also I find it weird that he's still ill. I don't think anyone seriously believes that Tiffany doesn't have Cancer. I would be more interested in hearing about her prognosis, and treatment plan. I don't know if she's asking her doctors the really tough questions that need to be asked. Either way, I think she needs a second opinion and a surgeon that makes her a priority.
I’m so confused… they were saying they couldn’t find any cancer on the last scans… so if it’s the case why do they categorise Tiff at stage 3 or 4. It’s always so confusing to follow.

T4B means the cancer has spread out of the bowel and into nearby organs. N1C means it’s in the fat surrounding local lymph nodes but hasn’t invaded the nodes themselves. CRM + means the tumor-free margin is less than or equal to 1mm. Tiffany, please seek a second opinion! I realize the cost of a private consult is very large and daunting but you need to have all this properly sorted and explained. This is stage four cancer and your life matters far, far more than how you’re being treated by the NHS. Sending love and hugs from Boston. 💗
Actually, the T has nothing to do with stage; it is an indication of how differentiated the cancer cells of the tumor are from other cells, with T1 being cancer cells that are not that different in appearance from normal cells. But maybe a different coding is used for this type of cancer than what I am used to so Tiffany’s own doctor should explain the coding.
@@sarahterry2140 yes this means III in France but maybe it’s different in US because sometimes there is little difference between a bad III and a good IV …?
I second the "Second Opinion." Now even if the surgeon finally sees her or the oncologist... why would I trust what he has to say now after he's dodged me all this time. Also, strange the surgeon would send her home without hospital stay after the biopsy - but then tell Tiffany, the bleeding is normal because he really scraped her, but assured her that next time she will have hospital stay. So, it sounds like he knew it would be a problem for her - plus, chemo can cause additional bleeding - and he let her go home?
I think this delaying tactic is terrible. Isn't 1 year of uncertainty enough? Nobody should be put off or relocated all the time. Especially not with such a life-threatening diagnosis. And they do that to you all the time! Even if you always defend your surgeon, I think it's unprofessional. Bleeding for weeks is also unacceptable. Why didn't he warn you immediately after (or even better before) the procedure?

There was a very disturbed individual on social media who lied about having cancer and being terminally ill. She was eventually found out. To put you in this catagory is ludicrous and very cruel. Sadly, another young girl dying of breast cancer was criticized by another RUclipsr and called a liar. She lost her GoFundMe and had to stop her cancer treatment, but before she did, she showed the horrible huge oozing tumor she had on her breast to prove she was not lying. She died shortly after. There is no limit to the cruelty of people, both the fakes and the ones that discredit the legitimate cancer victims.

It disappoints me that people would see you on here, being as transparent and open, the entire way, to say you are faking your diagnosis. Some people! You are so strong and those who have really been following are nothing short of proud of you and your strength. We got this. 💙
An unfortunate reality of dealing with hospitals and sickness is patients having to advocate for themselves because doctors etc have such a high workload. I see you are getting more confident asking questions and asking for what you deserve (ie. A copy of your results), so that's a huge positive. Is there a social worker at the hospital who could advocate for you?
When I was going through my cancer journey, because you couldn’t actually see where my cancer was situated, people questioned me too. There is nothing more disgusting than people disbelieving what you are going through 😢😢 Focus on getting better Tiff ❤❤ and thrive on the love being sent to you ❤ Every one at some time in life will suffer a health crisis ,I pray they don’t have to go through the crap you are having 🙏🏻🙏🏻🙏🏻🙏🏻

Stage III: Healthcare providers use this stage for large tumors that have spread to nearby lymph nodes, but not to other areas of your body. The TNM classification is T1-T4, N1-N3 and M0 The good news is M 0. means zero metastatic disease. T - tumor size N- lymph nodes M- metastasis I would ask if the tumor tissue is dead and can be removed safely. Ask how much tissue would be removed around tumor site and which organs like uterus ? praying for all of you

I've been fighting Triple Negative breast cancer for 5 years. It's an aggressive evil cancer and so my reaction to your waiting triggered that ugly companion that all who have been touched by cancer knows all to well, fear. There are several reasons why someone would message you negatively.. it ranges from their insecurity, jealousy, saddest mindset to evil intention to trouble someone and pleasure knowing they succeeded in disturbing you . Dear Tiffany, you know this percentage of people are minimal so don't address their foolishness...we love you and are praying for a miracle in your life.. Hope you don't focus anymore on the negative people and increase your focus on your health and the positive people around you... even the most beautiful rose has thorns.. so these situations make your even more precious! It had spread to my liver, and that was treated twice. Now a specialized Pt/Ct scan has determined that some lymph nodes in my chest are active, and that will be treated with chemo. I, too, thought I was going to have more surgery, but I’m not. It’s crazy, but I feel so thankful. ❤️
I can’t imagine your frustration, disappointment and confusion. Expecting answers, and then seeing a doctor who couldn’t give you answers you can be confident in. I am praying for you and hope that you can get the answers to your questions sooner rather than later. 🙏🏻❤️
Don’t worry about what other people say, I had lung cancer two years ago and had part of my lung removed, now I have two nodules, which they don’t know if they are cancerous or not. Will have scans and blood tests this week to see if anything else is going on. Keep fighting and don’t let the mean people bother you.

Tiffany you and Matt are SO right when you say that everyone's 'journey' with cancer, even with the same type of cancer, is just so individual - not only the course that the cancer itself takes, but also each individual's response to chemo, radiotherapy and other treatments. And I also so agree that often it's very difficult for oncologists to give you concrete answers, because they too just don't know quite how your body's going to respond, particularly to the more complex cancers. Also (this has turned into a stream of affirmations!), I totally agree that you should NOT think of changing hospitals, and in fact it is very unusual to do this is the NHS, because you would then be travelling miles outside your home area for each treatment. You are at one of the very best cancer hospitals and you trust your oncologist and your surgeon and they know your case very well... I think you're doing brilliantly and Matt is just a total gem, and very, very wise!

Hey tiff- I think people are just shocked that you can go from chemo not working at all, to suddenly your tumour disappearing in the space of a few weeks! Especially as there hasn’t been clear answers on if there actually is a tumour or if it’s gone? It seems like a pretty basic question to find the answer to, but yet your team hasn’t given you a straight answer.
Tumors can go through regression, ie shrinking, as a result of chemo and radiation. Sometimes they can disappear or sometimes they become fibrotic scar tissue. My tumor shrank significantly and also turned into scar tissue. There is research underway to determine if a tumor shrinks enough is it reasonable to delay surgery and monitor? If you can safely avoid surgery, that’s the way to go.
This is where people are confused... stage 4 is the size of the tumor... and shes still at stage 4, so basically what's happened is you can have a tumor be benign (non cancerous) and still call it a stage 4 tumor because it hasn't changed in size... the doctors are confused because her cancer markers are at zero, so basically according to all the recent tests, she is dealing with a very large invasive non cancerous growth. Which is still dangerous and she may still need that major surgery taking out everything in her pelvic region. Or perhaps she can live with keeping her bowels and her bladder and just dealing with the huge tumor, keeping an eye on it 🤔
People that understand the medical nuances during a medical crisis aren't shocked at all...this is what ppl going through cancer experience...limited information, Doctors with limited time, scans and biopsies not being cut and dry or black and white, different doctors telling you different things, this is far too common.
T4N1cM0 is NOT Stage 4 colorectal cancer!!! T4 indicates the size and local advanced growth through all layers of the colon, one lymphnode and the paracolic fat (N1c), but there are no distant metastases (M0).. I think that’s what certain people (5-10%) mean with their remarks that you are not stage 4. I don’t think they meant to call you a liar; it’s a misunderstanding from your side.
I can't speak for anyone else, but I have never doubted your truthfulness. The part I find so confusing is that you NEVER seem to know what is going on. I'm sure that many of your followers are cancer survivors going through similar cancers and similar diagnosis. There is a significant difference between stage 3 and stage 4. Stage 3 is advanced cancer, but may be treatable to the point of "curable " A Stage 4 diagnosis is defined as terminal, albeit many survive for years and years at this stage and advances are always being made to increase longevity and quality of life. Throughout my journey it has always been clear what stage I am. Your results seem so muddled . It's in your lungs, no it's not, it never has been, it's completely disappeared, WHAT??!!?? It has never been your job to read and interpret your scans. With all the uncertainty, and daily turmoil the cancer itself puts us through, I would have gone completely mad by now being as confused as you have been your entire cancer journey. With each of my scans, tests, bioosies, surgeries, multiple chemos, radiation, more surgeries, more tests, although i have often been terribly disappointed and fearful knowing the results, at least they were described succently, and i have not been confused. Nothing has been sugarcoated or ambiguous. You are an amazing human being. And so many of us look up to you. I pray that you are truly cancer free and can somehow get the clarification you deserve.
I agree. That is exactly what I am thinking. I too had cancer twice and always knew what stage my cancer was. Sometimes things changed drastic and I could have radiotherapy which was not an option weeks before. Chemotherapy worked way better than expected so plans changed. When I felt I didn´t get the answers I needed, I went to a private doctor and got every explanation I needed. One, or a few, private consultations are not that expensive.

I had a friend (no longer my friend) who lied about having terminal cancer, she insisted that she was told that surgery or chemo weren't a possibility (she never said why which should have been a red flag). She claimed that she would be dead within the next 3 months. Well, 3 months come and ago and she is still alive (which is good as I would not want to wish anyone death). She finally confessed that it was all lies. I cut all contact and haven't attempted to seek her out on any social media. She was part of the small percentage of people who lie about it, which people jump on to dismiss someone's genuine diagnosis and treatment plan. I had stage one renal cancer back in 2015 and I avoided putting too much of it out on the internet as I didn't want the reaction that you got. I think its good that you providing proof is going to be a one time thing, as they aren't worth constant explanations. I've not watched your channel for very long but I love your videos and your positive outlook through this. Thank you for sharing all of your struggle for yourself and your family. Stay amazing ^_^
Tiffany, you should be getting copies of ALL of your scans! You need to insist to have each and every one of them. These are YOUR records! I go to MD Anderson Cancer Center in Houston, TX every few months. As soon as my scans are done I have an appointment with my oncologist and she ALWAYS provides me with the written copies of my scans and labs. If you are being treated at the “best” hospital they should automatically be providing those to you each time too!

For my Head and neck cancer I was treated at the QUEENS MEDICAL CENTRE AND CITY HOSPITALS IN NOTTINGHAM. I had to best in Europe. My Oncologist Matt Griffin was brilliant with me he never missed one appointment with me. I spent months in hospital. Even when I went on to see him for 6 years he never missed one appointment with him. And David Grant who saw me at the start was Amazing too. He did original diagnosis. He told me speed is of the essence and his swift actions stopped me being stage 4. He is back working in America xx
Anyone following your journey can obviously see you are not faking so just ignore the trolls. In regards to your care and appointments I still don’t think you should be leaving confused. And unfortunately staging does matter because it tells you how advanced the cancer is and whether or not other organs are involved. Stage 3 vs 4 is a big deal. I know you say you love your surgeon but I’m sorry hun - I think he’s avoiding you. Also the surgeons are only focused on surgery not staging and treatments etc. I feel really bad that you are confused. Getting everything in letters and writing will hopefully help and I know you are scared to do your own research online but if you look at reputable medical websites then it might help you understand better and at the end of the day being educated about your own body is a good thing. Now I'm more than ever convinced that the doctors are avoiding you and giving you the runaround. It's crazy that you're not getting concrete answers or being able to meet with someone who knows the answers to your very sensible questions. Tiffany why feel the need to dedicate undeserving Time for a few "hate" comments while the vast majority who truly care are still left without answers that you have in your hands. You said don't panick I talked to the surgery team but never shared what they said. You have the markers in your hands and many have been asking about them yet no response. You have all the medical tests imaging and reports dear and a doctor cousin so why not ask her to clarify them. Why wait.. and I really hope you started therapy sessions (you didn't mention it) because you need to be as strong mentally as can be to take the right decisions in this period of your life. Hey Tiffany, Im a medical student and I wanted to help you understand the staging of colon cancer. What you have showed us- T4b N1c M0 is your TNM staging. It describes the size of the tumour (T), if the lymph nodes are affected (N) and if you have metastases in any other organs (M, 0 meaning that you have none). This TNM classification determines in what stage of cancer you are and in this case you are in stage III. Stage IV is for the patients that have distant metastases f.e. in their lungs. You can type "colon cancer staging" and read how different TNM means different stages. So the "T4" doesn't mean you are in the stage IV of colon cancer.

I'm not sure if you were genuinely asking if the letter was good news or if you just wanted some clarification, but if you're still unclear (you said you've spoken to your team so if they've already explained everything you can just ignore this! ), I can explain what all those confusing letters mean. T4B indicates that the tumor has penetrated the colon's wall and is either attached to or is moving in the direction of other surrounding organs or tissues. N1C indicates that the cancer has not yet reached the lymph nodes themselves but has spread to nearby fat or tissue. M0 indicates there's no detection of spread to distant organs. You've said that you value when we say things honestly and/or bluntly, but I can also understand that this comment could be overwhelming. Feel free to delete my comment if necessary, Tiff. @@sheouttapocket6637 No! the other markers are more important. Specifically, the (M) marker is the most important in this instance, which stands for metastasis. I'm not her oncologist and there are obviously things we aren't aware of but M0 indicates it hasn't spread to distant organs. The diagnosis markers she showed look like Stage 3C to me. M1A/M1B/M1C would be a Stage 4 indicator. I think Tiffany said they aren't worried about the spot in her lung but maybe the doctor she saw mistook that as Metastasis which is why she mentioned Stage 4.

Tiffany, I have been following your journey, and as a retired nurse I must admit the whole situation does seem a bit confusing.
I urge you to be more assertive during appointments, and not leave until you have the answers you need. Don’t worry about taking up your doctor’s time; it’s what he’s paid to do.
I find it helpful to make a question list and go through it systematically during an appointment.
I would certainly want the cancer stage explained fully and clearly, and the treatment options going forward outlined. Failing this I would seek a second opinion. There are lots of excellent oncologists out there.
Whatever you decide my thoughts and best wishes to you.

I'm not questioning you however I am questioning the cold shoulder the surgeon is giving you. He is still sick?? It's been quite a while. And he can call you, video chat, text?? In this day and age there are so many ways to communicate I find it hard to believe that it is impossible for him to communicate with you or any of his patients. I work in a hospital and a lot of the Drs I work with would at the very least call even if they were sickly.
My docter had Corona and was ill for six weeks, he wrote e mails to me and explained on the phone my diagnosis. My illness was a twisted ancle and I need a XRay, also nothing bad. I don't understand that the docter can not communicate with Tiff. when she has so a bad illness. Sorry about my englisch
When using TNM staging, T4 means locally advanced; N1c means cancer is present in surrounding fatty tissue; M0 means no distant metastasis (spread). It means you are not stage 4. However, it still means that cancer is present. For your own peace of mind and in order not to waste time, please get a second opinion. It honestly won’t hurt. Take care of yourself, stay positive (as always) and we are rooting for you.

@@Laura-gd4ku don’t know how old that information (T4bN1cM0) is. Recent scans may show NED (no evidence of disease) but it’s always good to check to confirm this.

You may use a different coding in the UK. In the US, size is captured in staging, while grading (the T system) speaks to how different the cancer cells are from normal cells when viewed under a microscope, with T1 for example being the least different and hence less aggressive than higher grades. It may also be that different cancers use different explanations for the grades.

Tiffany the cancer staging systems are complicated and hard to explain. It sounds like you are overall stage 3 but the tumour stage is T4b, the nodal stage is N1c and metastatic stage is M0 so no spread out the pelvis. You can look up TNM staging for colorectal cancer and you should be able find a table with each stage listed. T stage 4b means tumour has grown through the wall of the colon. N stage 1c means tumour is attached to nearby organs eg uterus, vagina.
Staging is very confusing and there are different systems.
Question to ask is whether your new TNM staging is pathological, based on recent biopsies or is it based on PET/CT/MRI scans. If tumour is still stage T4b then there is still cancer evident.
Some newish doctors or registrars may not be familiar yet with TNM staging and will find it hard to explain. I am a cancer research nurse so very familiar with it.
Also, it is confusing because in the subject line of letters they will usually always write your initial staging at diagnosis so you keep seeing that. It was hard to tell but I think that is what you showed not your new staging after biopsies and chemo. After you have your next scans ask 2 questions 1) what is my new TNM stage? and 2) what is my new overall stage of cancer?

I think it does matter alot whether its stage 3 or 4. It seems crazy that our best cancer hospital in the UK is so vague. Let's face it, if one in two is expected to have cancer at some stage then it's very worrying 🙈 . It doesn’t make sense that the surgeon is not available, also at the same time he told you to wait for surgery? Tiff FYI - no shouting - I promise 💐❣️ - T4b N1c MO = T4 is split into 2 stages, T4a and T4b: T4a means the tumour has grown through the outer lining of the bowel wall and has spread into the tissue layer (peritoneum) covering the organs in the tummy (abdomen) T4b means the tumour has grown through the bowel wall into nearby organs.
Hi Tiffany, I think why there’s some questions regarding your diagnosis is because of your chemo journey and then your biopsy showing no cancer. It does seem very incredulous
that you would go from TB4 to clear biopsies. The diagnoses you showed us with TB4 was for your right lung but wasn’t it determined that you didn’t have cancer in your lung after all? I think what people are asking for is your diagnoses for your colon cancer… I may be confused about this but I too am curious as to what your diagnosis was for your colon cancer. I am so happy for you if you are clear of cancer of course but it does seem a little confusing as to how you went from TB4 to clear biopsies. It’s not because we don’t believe you I think it’s because we’re a little bit confused. Can you show us your diagnosis for your colon cancer? I think this would clear up any doubts in anyone’s minds. I’ve been doing a bit of research myself on patients with your type of cancer at the stage that your cancer is at and apparently the wait and see approach is something fairly recent but it’s becoming more and more common. It tends to be done in cases where they feel that the cancer may no longer be operable. From what I’ve been reading, particularly after the level of testing and exploratory surgery that you’ve had, they want to give your body time to heal and also give your mind and emotions a chance to adjust. This is probably why they recommended therapy for you. I have never doubted for a minute that you have what you say you have, and I know there’s lots of different options in how doctors approach your type of cancer. I just feel strongly that you need to focus on what your reality is today and take advantage of this opportunity to get your mind in the right place to deal with all possible courses of action. I don't think anyone seriously believes that Tiffany doesn't have Cancer. I would be more interested in hearing about her prognosis, and treatment plan. I don't know if she's asking her doctors the really tough questions that need to be asked. Either way, I think she needs a second opinion and a surgeon that makes her a priority.
I’m so confused… they were saying they couldn’t find any cancer on the last scans… so if it’s the case why do they categorise Tiff at stage 3 or 4.

T4B means the cancer has spread out of the bowel and into nearby organs. N1C means it’s in the fat surrounding local lymph nodes but hasn’t invaded the nodes themselves. CRM + means the tumor-free margin is less than or equal to 1mm. Tiffany, please seek a second opinion! I realize the cost of a private consult is very large and daunting but you need to have all this properly sorted and explained. This is stage four cancer and your life matters far, far more than how you’re being treated by the NHS. Sending love and hugs from Boston. 💗
Actually, the T has nothing to do with stage; it is an indication of how differentiated the cancer cells of the tumor are from other cells, with T1 being cancer cells that are not that different in appearance from normal cells. But maybe a different coding is used for this type of cancer than what I am used to so Tiffany’s own doctor should explain the coding.

I second the "Second Opinion." Now even if the surgeon finally sees her or the oncologist... why would I trust what he has to say now after he's dodged me all this time. Also, strange the surgeon would send her home without hospital stay after the biopsy - but then tell Tiffany, the bleeding is normal because he really scraped her, but assured her that next time she will have hospital stay. So, it sounds like he knew it would be a problem for her - plus, chemo can cause additional bleeding - and he let her go home?

I think this delaying tactic is terrible. Isn't 1 year of uncertainty enough? Nobody should be put off or relocated all the time. Especially not with such a life-threatening diagnosis. And they do that to you all the time! Even if you always defend your surgeon, I think it's unprofessional. Bleeding for weeks is also unacceptable. Why didn't he warn you immediately after (or even better before) the procedure?

There was a very disturbed individual on social media who lied about having cancer and being terminally ill. She was eventually found out. To put you in this catagory is ludicrous and very cruel. Sadly, another young girl dying of breast cancer was criticized by another RUclipsr and called a liar. She lost her GoFundMe and had to stop her cancer treatment, but before she did, she showed the horrible huge oozing tumor she had on her breast to prove she was not lying. She died shortly after. There is no limit to the cruelty of people, both the fakes and the ones that discredit the legitimate cancer victims.

An unfortunate reality of dealing with hospitals and sickness is patients having to advocate for themselves because doctors etc have such a high workload. I see you are getting more confident asking questions and asking for what you deserve (ie. A copy of your results), so that's a huge positive. Is there a social worker at the hospital who could advocate for you?

You are such a blessing! I am going through cancer treatment, too. It’s an up and down journey. I had lung surgery two years ago (Neuroendocrine tumor). It had spread to my liver, and that was treated twice. Now a specialized Pt/Ct scan has determined that some lymph nodes in my chest are active, and that will be treated with chemo. I, too, thought I was going to have more surgery, but I’m not. It’s crazy, but I feel so thankful. ❤️

Don’t worry about what other people say, I had lung cancer two years ago and had part of my lung removed, now I have two nodules, which they don’t know if they are cancerous or not. Will have scans and blood tests this week to see if anything else is going on. Keep fighting and don’t let the mean people bother you.

Tiffany you and Matt are SO right when you say that everyone's 'journey' with cancer, even with the same type of cancer, is just so individual - not only the course that the cancer itself takes, but also each individual's response to chemo, radiotherapy and other treatments. And I also so agree that often it's very difficult for oncologists to give you concrete answers, because they too just don't know quite how your body's going to respond, particularly to the more complex cancers. Also (this has turned into a stream of affirmations!), I totally agree that you should NOT think of changing hospitals, and in fact it is very unusual to do this is the NHS, because you would then be travelling miles outside your home area for each treatment. You are at one of the very best cancer hospitals and you trust your oncologist and your surgeon and they know your case very well... I think you're doing brilliantly and Matt is just a total gem, and very, very wise!

Hey tiff- I think people are just shocked that you can go from chemo not working at all, to suddenly your tumour disappearing in the space of a few weeks! Especially as there hasn’t been clear answers on if there actually is a tumour or if it’s gone? It seems like a pretty basic question to find the answer to, but yet your team hasn’t given you a straight answer.

Tumors can go through regression, ie shrinking, as a result of chemo and radiation. Sometimes they can disappear or sometimes they become fibrotic scar tissue. My tumor shrank significantly and also turned into scar tissue. There is research underway to determine if a tumor shrinks enough is it reasonable to delay surgery and monitor? If you can safely avoid surgery, that’s the way to go.

This is where people are confused... stage 4 is the size of the tumor... and shes still at stage 4, so basically what's happened is you can have a tumor be benign (non cancerous) and still call it a stage 4 tumor because it hasn't changed in size... the doctors are confused because her cancer markers are at zero, so basically according to all the recent tests, she is dealing with a very large invasive non cancerous growth. Which is still dangerous and she may still need that major surgery taking out everything in her pelvic region. Or perhaps she can live with keeping her bowels and her bladder and just dealing with the huge tumor, keeping an eye on it 🤔
People that understand the medical nuances during a medical crisis aren't shocked at all...this is what ppl going through cancer experience...limited information, Doctors with limited time, scans and biopsies not being cut and dry or black and white, different doctors telling you different things, this is far too common.
T4N1cM0 is NOT Stage 4 colorectal cancer!!! T4 indicates the size and local advanced growth through all layers of the colon, one lymphnode and the paracolic fat (N1c), but there are no distant metastases (M0).. I think that’s what certain people (5-10%) mean with their remarks that you are not stage 4. I don’t think they meant to call you a liar; it’s a misunderstanding from your side.
I can't speak for anyone else, but I have never doubted your truthfulness. The part I find so confusing is that you NEVER seem to know what is going on. I'm sure that many of your followers are cancer survivors going through similar cancers and similar diagnosis. There is a significant difference between stage 3 and stage 4. Stage 3 is advanced cancer, but may be treatable to the point of "curable " A Stage 4 diagnosis is defined as terminal, albeit many survive for years and years at this stage and advances are always being made to increase longevity and quality of life. Throughout my journey it has always been clear what stage I am. Your results seem so muddled . It's in your lungs, no it's not, it never has been, it's completely disappeared, WHAT??!!?? It has never been your job to read and interpret your scans. With all the uncertainty, and daily turmoil the cancer itself puts us through, I would have gone completely mad by now being as confused as you have been your entire cancer journey. With each of my scans, tests, bioosies, surgeries, multiple chemos, radiation, more surgeries, more tests, although i have often been terribly disappointed and fearful knowing the results, at least they were described succently, and i have not been confused. Nothing has been sugarcoated or ambiguous. You are an amazing human being. And so many of us look up to you. I pray that you are truly cancer free and can somehow get the clarification you deserve.

I too had cancer twice and always knew what stage my cancer was. Sometimes things changed drastic and I could have radiotherapy which was not an option weeks before. Chemotherapy worked way better than expected so plans changed.
When I felt I didn´t get the answers I needed, I went to a private doctor and got every explanation I needed. One, or a few, private consultations are not that expensive.

I had a friend (no longer my friend) who lied about having terminal cancer, she insisted that she was told that surgery or chemo weren't a possibility (she never said why which should have been a red flag). She claimed that she would be dead within the next 3 months. Well, 3 months come and ago and she is still alive (which is good as I would not want to wish anyone death). She finally confessed that it was all lies. I cut all contact and haven't attempted to seek her out on any social media. She was part of the small percentage of people who lie about it, which people jump on to dismiss someone's genuine diagnosis and treatment plan.
I had stage one renal cancer back in 2015 and I avoided putting too much of it out on the internet as I didn't want the reaction that you got. Tiffany, you should be getting copies of ALL of your scans! You need to insist to have each and every one of them. These are YOUR records! I go to MD Anderson Cancer Center in Houston, TX every few months. As soon as my scans are done I have an appointment with my oncologist and she ALWAYS provides me with the written copies of my scans and labs. If you are being treated at the “best” hospital they should automatically be providing those to you each time too!
I suppose in state you guys have medical insurance?
We have our dear national health which is overwhelmed with high demand.

For my Head and neck cancer I was treated at the QUEENS MEDICAL CENTRE AND CITY HOSPITALS IN NOTTINGHAM. I had to best in Europe. My Oncologist Matt Griffin was brilliant with me he never missed one appointment with me. I spent months in hospital. Even when I went on to see him for 6 years he never missed one appointment with him.
And David Grant who saw me at the start was Amazing too. He did original diagnosis. He told me speed is of the essence and his swift actions stopped me being stage 4. He is back working in America xx

Anyone following your journey can obviously see you are not faking so just ignore the trolls. In regards to your care and appointments I still don’t think you should be leaving confused. And unfortunately staging does matter because it tells you how advanced the cancer is and whether or not other organs are involved. Stage 3 vs 4 is a big deal. I know you say you love your surgeon but I’m sorry hun - I think he’s avoiding you. Also the surgeons are only focused on surgery not staging and treatments etc. I feel really bad that you are confused. Getting everything in letters and writing will hopefully help and I know you are scared to do your own research online but if you look at reputable medical websites then it might help you understand better and at the end of the day being educated about your own body is a good thing. Everyone is praying and rooting for you. I just really feel like your team are letting you down. Good luck sweetheart. Really hope it all goes well 🙏🏻♥️🙏🏻

Hi Tiffany, I'm so sorry that you felt forced to prove yourself because of some negative people on here. I wonder if some of those people didn't actually mean that they think you are lying, but maybe meant that they think the doctors got it wrong? I don't know I just can't fathom anyone could doubt your honesty and sincerity.

Now I'm more than ever convinced that the doctors are avoiding you and giving you the runaround. It's crazy that you're not getting concrete answers or being able to meet with someone who knows the answers to your very sensible questions. I think you need to demand a meeting with the oncologist and surgeon together and find out what is going on.
She hasnt said much about what they tell her so it may not be that they arent saying anything

I’m so upset seeing this video, I can’t believe how nasty some people can be online, I lost my husband to Covid in December last year, his cancer was in remission, someone online did a video stating my Husband wasn’t that Ill, what’s wrong with people, your such a beautiful person and so brave , please don’t allow anyone who is negative to you online, get to you, they are not worth it , sending you so much love ❤❤❤❤

It’s so confusing when you have one doc saying one thing and another doc says another thing. It’s frustrating too! I’m so sorry you’re going through this.
Tiffany why feel the need to dedicate undeserving Time for a few "hate" comments while the vast majority who truly care are still left without answers that you have in your hands.
You said don't panick I talked to the surgery team but never shared what they said. You have the markers in your hands and many have been asking about them yet no response. You have all the medical tests imaging and reports dear and a doctor cousin so why not ask her to clarify them. Why wait.. and I really hope you started therapy sessions (you didn't mention it) because you need to be as strong mentally as can be to take the right decisions in this period of your life.
Kindly accept these words not as shouting or anything but because we want the best outcome for such a lovely young woman we decided to follow her journey and live this rough patch in her life along with her. 💙
